 Reports To: Director of Home Care 
 FLSA Status: Non-Exempt $23 to $25 per hour 
 Schedule: Full-time, Monday-Friday, 12:00 p.m. – 8:00 p.m. with on call rotation

Summary

Glenmeadow At Home is seeking a Client Relations and Scheduling Supervisor to support staff, clients, and families through real-time problem solving, scheduling, and service oversight. This role oversees daily staffing coordination, supports caregivers, and ensures continuity and quality of care while collaborating with nursing and leadership to maintain compliance and advance Glenmeadow’s mission.

Essential Duties & Responsibilities

Staffing & Scheduling

  • Manage daily staffing schedules to ensure consistent coverage and continuity of care.
  • Address call-outs and last-minute schedule changes, arranging real-time coverage as needed.
  • Maintain accurate scheduling and call-out documentation.
  • Serve as on-call scheduler every other week, providing after-hours support.
  • Support staffing consistency, labor cost controls, and provide direct coverage when necessary.

Supervision & Staff Support

  • Supervise and support Glenmeadow At Home caregiving staff, serving as a field resource for care delivery and emergent needs.
  • Reinforce professionalism, communication, safety, and documentation standards.
  • Support onboarding, training, and teamwork to promote accountability and high-quality service.

Client Care & Service Coordination

  • Assess and evaluate client care needs in collaboration with the residents, their families, nursing, and care management.
  • Assist in developing, updating, and maintaining service plans in compliance with state regulations.
  • Promote client independence, dignity, and engagement in activities of daily living.

Administrative & Compliance Responsibilities

  • Ensure timely and accurate completion of documentation, including incident reports and care-related records.
  • Enter and maintain records in electronic systems (e.g., Yardi).

Knowledge, Skills, and Abilities

  • Strong organizational and scheduling skills.
  • Ability to lead and support staff with professionalism, empathy, and clarity.
  • Effective written and verbal communication skills.
  • Strong problem-solving and decision-making abilities, particularly in time-sensitive situations.
  • Ability to manage competing priorities and adapt to changing needs.
  • Commitment to confidentiality, resident rights, and ethical practice.
  • Ability to stand, walk, bend, reach, and lift up to 40 pounds.

Experience and Education  

  • High school diploma or GED required; post-secondary education in healthcare or human services preferred.
  • Minimum of five (5) years’ experience working with older adults in a healthcare or home care setting.
  • CNA or HHA certification required, or equivalent health care education (Prior supervisory, lead, or scheduling experience strongly preferred.
  • Proficiency with electronic health records and scheduling systems (e.g., Yardi).
