We are seeking a compassionate Wound Care/Foot Care Nurse Practitioner to provide specialized wound care services to patients in long-term care facilities on a part-time basis. Key Responsibilities:

  • Provide foot care and/or wound care to residents in long-term care facilities
  • Conduct thorough wound assessments and develop personalized care plans for each patient
  • Perform wound debridement, dressing changes, and other wound care procedures as needed
  • Continuously monitor and assess patients’ progress in wound healing and foot health
  • Educate patients and facility staff on effective wound care techniques, prevention strategies, and foot care best practices
  • Travel to designated nursing homes in your area
  • Communicate regularly with key facility staff, physicians, and healthcare professionals to ensure seamless care delivery and optimize patient outcomes
  • Maintain accurate and up-to-date patient records and documentation
  • Manage pain related to wounds to ensure patient comfort
  • Maintain flexibility while seeing patients during weekday working hours

Qualifications:

  • Master’s or Doctoral degree in Nursing from an accredited program
  • Current Nurse Practitioner license in Kentucky
  • Certification in Wound, Ostomy, and Continence (WOCN) is preferred but not required
  • Minimum of 2 years of experience in wound care and/or foot care
  • Strong clinical skills in assessment and treatment of wounds and foot conditions
  • Independent, organized, and motivated to provide exceptional patient care
  • Excellent communication and interpersonal skills
