All employees, including the Lead Program Instructor, are expected to model and promote the organization’s mission and values in every aspect of their work.  Position Summary:  Lead Senior Instructor provides administrative day program supports, to ensure documentation is completed by Instructors to show the progress or lack of progress towards meeting the individuals' ISP goals for specific waiver services. Participates in all scheduled ISP meetings to ensure the day program services meet the needs of the individual. Conduct daily progress notes audits and monitor individuals' treatment plan. Assist with supervision and coordination. of department staff and functions. The job requires 60% administrative work, 30% direct care, and 10% transportation, and other duties as assigned.  Essential Job Duties and Responsibilities:  Ensures appropriate staffing within the program in order to remain in compliance with all regulatory requirements and to provide effective care to individuals.  Monitor staff to improve quality and quantity of work performed and provides training and development opportunities as appropriate.  Demonstrate thorough knowledge of the services the Day Program offers.  Aids in the proper maintenance of records and fi1es of individuals and operations.  Supports the Program Manager and Program Instructors. Recommends or initiates disciplinary actions according to applicable rules and policies.  Works to maintain and improve continuous customer service relations with managers and other employees. Create and maintain a high-performance environment characterized by positive leadership and a strong team orientation.  Aids in the supervision of other employees to help maintain a smooth operational environment. Supports Program Manager and employees through appropriate oral and written communication and people skills. Create and maintain a high-performance environment characterized by positive leadership and a strong team orientation.  Assist with training new employees, as well as experienced workers in areas such as agency policy, department procedures and .  Participate in developing and implementing agency administrative policy.  Supports in planning and executing daily activities of individuals with other staff and Program Manager.  Communicates regularly with the Program Manager.  Maintain relationship with other collaboratives to ensure that everyone is working towards the successful completion of the ISP goals for the individual.  Actively learns about the interests. Goals, preferences, support needs and skills of each person served through direct interaction, observation, insights of staff, team members, and family members  Actively attend all ISP and SIS meetings as a representative of the agency, and ensures that all ISP's, HRST's, and BSP's are up to date and in case record.  Assists in coordinating interdisciplinary treatment team meetings and assist in developing individuals ISPs and assures plans are implemented by staff from multiple disciplines so that identified goals are met and required documentation and tracking of ISP goals is completed.  Effectively conducted periodic meetings (both regular and called meetings, as necessary) of individuals to make any changes necessary to the individuals ISP.  Directs interdisciplinary team to identify day program goals based on individual's needs and day program services.  Disburse and posts information regarding individuals' treatment plans.  Monitors the implementation of individual individuals plan through frequent rounds and observation, as necessary.  Ensures collection and timely submission of data, reports, forms and other pertinent information as required to meet individual needs of individuals.  Effectively monitors the individuals' progress towards achieving goals, objectives, and amends plan as needed.  Ensures individual documentation and records requirements are being met. Conduct monthly individual record reviews; responsible for documenting outcomes and developing a plan of action.  Ensures activity notes are being documented to show provision of services, as indicated on the current ISP and correspond to goals on the current ISP.  Ensures that all required individuals support documentation is correctly and accurately completed and submitted daily.  Ensure the safety and well-being of individuals while in the program care.  Ensure safe transportation to individuals served by the Day Program  Accompany and transport individuals to participate in recreation and community outings.  Maintain a safe driving record and comply with all local traffic regulations.  Obtain 16 hours of annual training as required by InCommunity Stakeholders.   On duty for the hours assigned and implement planned scheduled.  Attend regularly scheduled meetings.  Successfully completes Proxy Caregiver Training, both general training and work site specific training.  Other duties as assigned.  QualificationsEducation, Certifications & Experience Requirements  High School Diploma and six months’ experience in working in human services or related field.  Work Environment & Physical Demands  The work environment characteristics described here are representative of those an employee encounters while performing the essential functions of this job.
