The Independent Living Case Coordinator ensures that individuals supported by IMPACT in the Independent Living department have adequate and continuous personalized supports that enable them to participate fully in life opportunities in their home, at work and in the community. The Coordinator acts as a liaison between DDS case managers and/or guardians, ensuring communication is fluent and proactive, preparing reports and representing IMPACT at IP meetings and the development and maintenance of records in Therap.

The Independent Living Case Coordinator has a dual role and completes their individualized work with the individuals’ receiving support, whether that is in their own home or at our main location. The case coordinator completes such work, face to face, and ensures appropriate reimbursement is secured. Secondly, the Coordinator assists the Director of Independent Living with monthly billing, record reviews, and provides overall administrative support.

Qualifications & Training:

  • Minimum of a High School Diploma/GED 
  • Valid CT Driver’s License 
  • Typically, 3-5 years of job experience. Experience working with individuals with ID/DD and physical disabilities exhibiting challenging medical, physical, and behavioral needs required for some programs. Will train individuals with a genuine commitment and interest in working with adults with ID/DD. 
  • Willingness to work a flexible schedule; including different programs as needed. 
  • Ability to maintain assigned work hours. 
  • Have or be able to obtain CPR/First Aid, PMT, and DDS Medication Administration Certification, as required. 
  • Excellent computer skills, including a high degree of proficiency in Microsoft Word, Excel, Outlook, and PowerPoint. 
  • Experience/proficiency in Therap and EVV (Electronic Visit Verification) strongly preferred.
