Description
  • Provide opportunities for community integration.
  • Document all day-to-day activities and ensure required documentation is completed.
  • Excellent written and verbal communication skills to engage with individuals, families, coworkers, and outside agencies
  • Assist clients in maintaining a clean home
  • Ensure  individuals receive their medication and document accurately
  • Assist individuals with meal preparation
  • Take individuals to their doctor appointments and ensure documentation by providers is completed.
  • Work to implement the goals of the individuals.
  • Engage with ongoing training requirements 24 hours of annual training required. 
  • Exhibit a positive attitude
  • Be flexible and a team player
  • Promote independence and an everyday life
Requirements
  • High school diploma or GED equivalent
  • Valid driver's license
  • CPR/AED/First Aid certification preferred (training available)
  • Medication Administration preferred (training available)
  •  Physical and TB test 
  • Ability to pass a background check and, if applicable, FBI clearance.
  • Ability to stand for long periods
  • Ability to handle stressful situations
  • Ability to lift, push, and pull 50 pounds
  • Ability to complete one-person transfers with a wheelchair
