Position Summary

The Wellness Outreach Clinician serves on the Outreach Health Coach assisting with health assessments, education, and consultations to Outreach customers. The Wellness Clinician assists with the facilitation of health fairs, wellness programs, classes and education events. The Wellness Clinician may also conduct interviews, participate in screenings, and/or review results of health or wellness plans with patients of the Outreach department.

Responsibilities

Performs assessment to include psychosocial, sensory perception, function/mobilization, skin integrity, age related changes and nutritional needs. Obtains patient history, and patient safety checklist, i.e., high risk for fall within scope of licensure.

Develops relationships with participants by assisting them with the process of working towards better health providing support, encouragement, and education.

Documents identified teaching needs and implements plan including patient and/or significant other. Discusses appropriate screenings (mammogram, colorectal, cervical, and dental) with participants as appropriate.

Review lifestyle profile with each participant and gather information about medication, pertinent medical history, social and life styles.

Uses knowledge and skills to discuss wellness with participants. Topics may include health related behaviors, medications, medical history, family history, health concerns and health promotion resources.

Ability to collect specimens and perform simple finger pricks, if necessary. Obtains vital signs, height and weight of participants. 

Ability to use AED if needed.

Minimum Requirements

High School and 1-2 years of formal training/education.

Strong preference for a Bachelor or Master's degree in allied health or related field.

American Heart Association BLS is required.

Pay Range

$24.69 - $35.99

Starting pay in the Outer Banks Hospital/Outer Banks Medical Group market may be higher to reflect local market competitiveness.

Shift expectations: Monday thru Friday with weekend and nights

The Outer Banks Hospital is a full-service critical access hospital offering a wide range of inpatient and outpatient services. The 21-bed facility is designed to meet the needs of the coastal community, whose year-round population of approximately 35,000 swells to about 350,000 in the summer season.
