Pay Rate: $40.00 per hour + Benefits Package
Hours: Monday-Friday 8:30AM-3:00PM (32hrs)

OVERVIEW OF PRIMARY RESPONSIBILITIES:
The PROS team works collaboratively in a dynamic, person-centered, evidence-based program that provides a broad range of services to adults with mental illness to help them achieve goals for work, education, relationships, socialization, living, and health and wellness. Other team members include a program director, psychiatrist, recovery counselors, employment specialists, mental health counselors, graduate student interns, and clinical and administrative support staff. The RN serves as the health expert on the team helping to integrate medical and psychiatric care.Â
PRIMARY FUNCTIONS:
- Conduct health assessments to determine health related problems and areas of focus
- Counsel participants individually and in groups on medical self-care
- Develop and lead health education classes on topics such as diabetes, heart disease, substance use, asthma, nutrition, wellness, smoking cessation, and medication self-management
- Administer injections and assist with medication packaging to improve adherence
- Coordinate laboratory services and reports
- Refer to and maintain contact with primary care providers
- Document all services and progress notes in electronic health record
ADDITIONAL FUNCTIONAL / ORGANIZATIONAL SUPPORT:
- Order and store medical supplies
- Attend clinical and administrative team meetings
- Other duties as assigned
QUALIFICATIONS AND ATTRIBUTES:
- Excellent computer, writing and oral communication skills essential. Â
EDUCATION AND EXPERIENCE:
- NYS license and current registration to practice as a Registered Nurse
- Knowledge of mental illness and psychotropic medications strongly preferred.

- Must be capable to sit or stand in front of a computer for long-periods of time
- Able to work in open space floor plan
- Must be capable to move throughout work day and follow people served throughout community

- Occasional lifting of > _25+ pounds
