We are currently seeking a Care coordination Team Leader to work at our 400 Broadway location. 


Position Summary: 

Responsible for providing psychosocial related support to clients living with HIV who are experiencing challenges with their HIV care. Leads initial care/service plan, conduct assessment on social network diagram, individual counseling, booster session, as well as group counseling. Responsible to collaborates with the Care Coordination Programs Manager and the Care Coordinator to promote overall clients’ well-being and quality of life.

 

 

The major duties of the position include, but are not limited to:

Program Management & Training –

  • Provide day-to-day oversight of the Be Into Health program and implement the program services based on policies as required.
  • Work with the Care Coordination Programs Manager to develop and monitor program deliverable.
  • Leads interdisciplinary client related discussion and planning.
  • Provide supervision to Care Coordinator by face-to-face case discussion and contribute to performance reviews.
  • Prepare and maintain administrative and business reports and records including staff and client travel and monthly expenses.


Direct Care –

  • Conduct social network activity; initial client care plan development and/or update; counseling sessions, and booster sessions with clients and support confidants.
  • Under the leadership of Care Coordination Programs Manager, to create curriculum for and to facilitate psychosocial group counseling and individual counseling for the program’s clients and collaterals.
  • Ensure that issues/concerns encountered or raised at/by the clients are resolved; to ensure smooth operation of the program, including troubleshoot any crisis with clients.
  • Document, track, and monitor services provided. 
  • Maintain accurate records of services rendered. 


Compliance –

  • Create and maintain up to date supervision logs.
  • Ensure assigned program staff maintain accurate records on the services and activities such as program registration, client-related pertinent documents, client’s interactions in designate database and reporting structures in accordance to the protocols and standards of Apicha’ s Department of Support Services within required time frame.
  • Ensure all service requirement under assigned team are met and client adhere to program guidelines as required.

 

Quality Assurance & Improvements –

  • Lead program staff in quality assurance and quality improvement activities.
  • Working as a part of the unit at the quality assurance and continue quality improvement (CQI) tasks. 
  • Collects, prepares, and reports of services and activities completed in a timely manner.
  • Participates internal and external meetings including assists as requested and appropriate.

 

Collaboration & Outreach –

  • Collaborates with external agencies and internal staff including clinical team to manage care coordination resources and partnership to target the neediest PLWHA for service.
  • Filed and response to telephone, e-mail, and other requests for information.
  • Assist or provide back-up to Care Coordinator.
  • Represent the agency at program related provider meetings as well as various venues to promote agency services.




Qualifications:

  • Advance degree in mental health counselling or social work with two (2) year of qualifying experience required.
  • Must hold a valid New York State license to provide mental health services (LMSW) or equivalent (e.g., LSW, LCSW, LMHC, etc.)
  • Experience supervising paid staff a plus.
  • Qualifying experience including working with target populations: individuals living with HIV/AIDS who are virally unsuppressed, Serious Mental Illness, and/or serious substance use disorders (SUDs).
  • Appreciation of and commitment to Apicha’s mission and values.
  • Excellent MS Office skills (Outlook, Word, Excel, PowerPoint). Familiarity with AIRS, eSHARE and/or eCW preferred.
  • Must possess ability to interact with all levels of external and internal customers with a high level of professionalism and tact in representing Apicha CHC.
  • Strong organizational skills with ownership mentality.
  • Strong verbal and written communication skills – for both internal and external communications.
  • Self-motivated, outcome oriented, problem solver strongly preferred.
  • Excellent customer service skills to assist in resolving member complaints.
  • Ability to work effectively in both a collaborative as well as individual setting.
  • Ability to work flexible hours including evenings and weekends at various venues required.
  • This is an on-site position.
