Job DetailsJob Location: Health Homes - Staten Island, NY 10301Position Type: Full TimeEducation Level: High School/GEDSalary Range: $22.42 - $24.83 HourlyJob Shift: DayWork Schedule: Monday- Friday, 9:00 a.m. - 5:00 p.m. SUMMARY: The Care Coordinator is responsible, under the supervision of the Program Director/Care Manager to serve a designated caseload of clients in our Health Homes Program. Day-to-day activities focus on the provision of services so all needs presented by clients are being met. Will play an integral part in the care management team. RESPONSIBILITIES: Provide intensive care coordination activities for clients. Assuring that members have access to, engages in, and retain needed services as defined in the member's care plan. Such services may include Acute Medical Care, Primary Medical Care, Preventative Medical Care Services, Home Health Care, Behavioral Health Services, Community Social Support Services, Housing & State, and Federal entitlements. Advocate for client services by phone and in-person as needed. Perform home visits and follow-up to monitor services and client status. Participate in case conferences and training. Maintain relationships with service providers. Maintain accurate and timely documentation including all documents included in client charts, records, and billing activity logs. Attend supervision with the Care Manager as per agency policy.  Qualifications Requires a Bachelors Degree or a High School Diploma with 2 years of care management experience. Experience working with the homeless, and HIV populations required. Bilingual preferred, with good verbal skills in Spanish.      Must have exceptional communication skills both orally and in writing, with individuals and groups regarding complex and sensitive issues.      Clean valid driver's license required.         Â
