Responsibilities:

  • Assesses the nutritional status of residents through documentation in the medical record in a timely manner. Uses parameters such as anthropometric measurements, nutrition focused physical assessment and interpretation of laboratory values.
  • Develops and revises a nutrition care plan based on information from the resident, medical record, family and health care team members.
  • Incorporates current evidence-based research into practice.
  • Adheres to facility confidentiality, HIPAA regulations, and resident rights policies.
  • Complies with regulatory agency standards, including federal, state and JCAHO as evidenced by successful completion of surveys with no deficiencies.
  • Completes all required documentation, including assessments, MDS 3.0, reports and logs in a timely, professional, and thorough manner.
  • Communicates effectively with all team members, residents and families through conferences, individual consultation, meal rounds, and committee participation. This includes telephone, written, and electronic communication. Emails to be checked frequently throughout each day, and, if required, responded to as soon as possible.
  • Notifies the CNM and/or FDS if duties are unable to be performed due to illness or work constraints
  • Orients, mentors and trains staff and interns, as applicable. Participates in Departmental health initiatives including health fairs, promotions and educations via bulletin boards, in-services or other educational media.
  • Advises department on resident care and food service issues in a timely manner. Completes test trays and tray audits as required.
  • Plans regular and modified menus based on current diet manual.
  • Follows facility and department infection control policies and procedures.
  • Participates in facility-wide and department Performance Improvement Program to improve processes, resident care and professional skills.

Education & Experience

  • Bachelor's degree from four-year college or university; minimum one (1) year work experience as a dietitian preferred.

 

License Requirements:

  • Registered Dietitian (Commission on Dietetic Registration).
  • State Licensure, as required
