Memory Care LPN

Job Description:

1. Rotating weekends, day/night shift 
2. The LPN provides clinical oversight, medication management, resident assessments, care coordination, staff supervision, and ongoing monitoring of residents. 
3. The LPN works collaboratively with caregivers, medication technicians, physicians, families, and other healthcare professionals to promote resident safety, dignity, independence, and quality of life. 
4. In the essential duties and responsibilities – provide direct nursing care to residents in accordance with applicable laws, regulations, facility policies and individualized care plans
5. Perform medication reconciliation, medication reviews, controlled substance counts, and medication documentation as required. 
6. Promote a safe environment and identify potential risks related to falls, elopement, medication access, infection control and resident behaviors
7. Respond appropriately to emergencies and initiate appropriate interventions within the scope of practice 
8. Communicate effectively with residents, families physicians, pharmacies, home health agencies, hospice providers and other members of the healthcare team
9. Assist in training and monitoring of medication administration
10. Perform resident assessments
11. Supervise and support care staff/ med-techs in accordance with applicable laws, regulations, & facility policies
12. Maintain ongoing compliance with chapter code 2600 regulations

Shift Differentials:

Evenings: $1 extra!

Nights: $2 extra!
