At Home Health Care, Inc., we're hiring an RN Visit Nurse to provide skilled nursing visits throughout St. Paul and surrounding communities.

We're looking for a nurse who doesn't need someone standing over their shoulder to provide excellent care.

You're trusted to walk into a patient's home, complete a strong clinical assessment, recognize concerns, provide appropriate skilled care, educate the patient, and determine when something needs to be escalated.

But you're the nurse in the home.

Your assessment matters.

And your ability to manage your day matters.

If You Like Independence, You May Love Visit Nursing.

This isn't the right nursing environment for everyone.

You won't have a charge nurse ten feet away.

You won't have another RN sitting at the nurses' station beside you.

And you won't spend your entire shift on one hospital unit.

You'll be out in the community, moving between patient homes and making decisions throughout your day.

For the nurse who is organized, confident, clinically strong, and self-directed, that independence can be one of the best parts of the job.

What Your Visits Could Include

You'll provide skilled nursing care to patients throughout the St. Paul territory, including:

  • Comprehensive RN assessments
  • Skilled home health nursing visits
  • Wound assessment and wound care
  • Medication reconciliation and management
  • Injections and other skilled nursing procedures
  • Monitoring acute and chronic conditions
  • Patient and caregiver education
  • Identifying changes in condition
  • Communicating clinical concerns with physicians
  • Coordinating with PT, OT, SLP, social work, and other clinicians
  • Supporting safe transitions home following hospitalization or rehabilitation
  • Helping prevent avoidable emergency department visits and rehospitalizations
  • Completing timely documentation in Homecare Homebase (HCHB)

Your Job Isn't Just to Complete the Visit.

A great Visit Nurse looks beyond the immediate task.

Why isn't the wound improving?

Is the patient actually taking the medication correctly?

Does the caregiver understand the discharge instructions?

Has the patient's mobility changed?

Is there something happening in the home that's making recovery more difficult?

Could something you noticed today prevent an emergency tomorrow?

That's where clinical experience becomes incredibly valuable.

You're not simply providing a service.

You're assessing the entire situation and helping determine what that patient needs to remain safely at home.

Who We're Looking For

We're looking for an RN who can be trusted with both the clinical responsibility and independence that come with field nursing.

You'll need:

  • An active Minnesota Registered Nurse (RN) license
  • Previous experience in home health, hospital, acute care, rehabilitation, skilled nursing, or another clinical environment
  • Strong clinical assessment and critical-thinking skills
  • Confidence working independently
  • Excellent patient and caregiver communication
  • Strong organization and time-management abilities
  • Ability to recognize and appropriately escalate changes in condition
  • Reliable transportation and willingness to travel throughout the St. Paul service area

Home health experience is certainly valuable.

But if you've built strong clinical skills somewhere else and you're ready for more independence, don't assume you need to spend the rest of your career inside a hospital or facility.
