Our campus provides Independent Living, Assisted Living, Memory Care, Skilled Nursing, and Hospice services. Beyond Hospice, a service of Episcopal Homes of Minnesota, is seeking an experienced and compassionate Registered Nurse Case Manager to provide end-of-life care and support to patients and their families. This is a great opportunity for an RN who enjoys working independently while being part of a supportive interdisciplinary team. Pay & Schedule Pay: $37.44–$56.66 per hour, based on experience Schedule: Monday–Friday, 8:00 a.m.–4:30 p.m. Weekend Rotation: Every 6th weekend What You’ll Do Complete initial and ongoing nursing assessments and develop individualized plans of care. Provide skilled nursing care focused on comfort, symptom management, and quality of life. Administer medications and treatments and complete medication reconciliation. Serve as Case Manager for assigned patients, coordinating all aspects of hospice care. Educate and support patients, families, and caregivers throughout the hospice journey. Coordinate medications, medical equipment, supplies, and community resources. Collaborate with physicians, social workers, chaplains, therapists, and other members of the interdisciplinary team. Communicate changes in patient condition and obtain provider orders as needed. Supervise and support Hospice Home Health Aides and LPNs. Maintain timely and accurate clinical documentation. Participate in interdisciplinary team meetings and the on-call rotation. Follow CMS, Medicare, and State of Minnesota hospice regulations and participate in QAPI initiatives. Build relationships with contracted facilities and community providers and provide hospice education as needed. Qualifications Required Graduate of an accredited school of nursing. Current Minnesota Registered Nurse (RN) license. Strong assessment, communication, critical thinking, and documentation skills. Valid driver’s license, reliable transportation, and current auto insurance. Preferred Bachelor’s degree in Nursing (BSN). Two years of acute care nursing experience. Two years of recent hospice or home health experience. Physical Requirements Ability to lift, push, or pull up to 50 pounds. Ability to stand, walk, bend, reach, and assist with patient mobility. Ability to provide care in patients’ homes, hospice settings, and community locations. Comfortable supporting patients and families through emotionally sensitive situations.
