Description

Plaza Community Living Center is seeking a compassionate and dedicated Speech Therapist to provide high-quality speech, language, communication, and swallowing services. 


Key Responsibilities

  • Assess, diagnose, and treat speech, language, communication, cognitive, and swallowing disorders.
  • Develop and implement individualized treatment plans based on each patient’s needs and goals.
  • Conduct screenings and evaluations to identify patients who may benefit from speech therapy services.
  • Monitor patient progress and modify treatment plans as needed.
  • Collaborate with nursing staff, physicians, and other healthcare professionals to provide comprehensive patient care.
  • Educate patients, residents, and families regarding communication and swallowing disorders and treatment recommendations.
  • Maintain accurate and timely documentation in accordance with facility and regulatory requirements.
  • Provide services across two facilities as needed.


Requirements

Qualifications & Skills

  • Current Speech-Language Pathologist license/certification as required by the state.
  • Strong clinical knowledge and assessment skills.
  • Ability to work effectively with patients with a variety of needs and personalities.
  • Excellent interpersonal and communication skills.
  • Professional, compassionate, and patient-centered approach.
  • Ability to work independently while collaborating effectively with a multidisciplinary team.
  • Commitment to continued professional growth and staying current with treatment practices.
