RN Manager – Care Management

Location: Petaluma, California
Job Type: Full-Time
Schedule: 8-Hour Day Shifts

Make an Impact in Care Management

We are seeking an experienced RN Manager – Care Management to lead and support care coordination initiatives within an acute care environment. This is an excellent opportunity for a nursing leader who is passionate about improving patient outcomes, coordinating complex care, and developing high-performing teams.

Key Responsibilities

  • Lead and support care management and care coordination activities across the patient care continuum.

  • Collaborate with patients, caregivers, nurses, physicians, social workers, payers, and other healthcare professionals.

  • Facilitate effective transitional care management and promote continuity of care.

  • Address complex patient needs, including social determinants of health, behavioral health, substance use, and resource barriers.

  • Support quality, patient safety, regulatory compliance, and operational goals.

  • Provide leadership, guidance, and mentorship to care management staff.

Required Qualifications

  • Bachelor’s Degree in Nursing (BSN)

  • Active California RN License upon hire

  • Minimum 5 years of healthcare experience

  • Minimum 3 years of Case Management experience in an acute care/inpatient hospital setting

  • Minimum 1 year of healthcare leadership experience

Preferred Qualifications

  • Master’s Degree in Healthcare or a related field

  • CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification
