Make an Impact in Care Management
We are seeking an experienced RN Manager – Care Management to lead and support care coordination initiatives within an acute care environment. This is an excellent opportunity for a nursing leader who is passionate about improving patient outcomes, coordinating complex care, and developing high-performing teams.
Key Responsibilities
- Lead and support care management and care coordination activities across the patient care continuum.
- Collaborate with patients, caregivers, nurses, physicians, social workers, payers, and other healthcare professionals.
- Facilitate effective transitional care management and promote continuity of care.
- Address complex patient needs, including social determinants of health, behavioral health, substance use, and resource barriers.
- Support quality, patient safety, regulatory compliance, and operational goals.
- Provide leadership, guidance, and mentorship to care management staff.
Required Qualifications
- Bachelor’s Degree in Nursing (BSN)
- Active California RN License upon hire
- Minimum 5 years of healthcare experience
- Minimum 3 years of Case Management experience in an acute care/inpatient hospital setting
- Minimum 1 year of healthcare leadership experience
Preferred Qualifications
- Master’s Degree in Healthcare or a related field
- CCM (Certified Case Manager) or ACM (Accredited Case Manager) certification
