The Home Trainer provides needed support, supervision, coaching, mentoring and training to developmentally disabled individuals in their home and work environment.


  • Maintains detailed daily IDD service notes and other documentation required by MS Department of Mental Health. Performs all required work as assigned in working with residents. Ensures daily outcomes are met and follows the Plan of Services and Supports for each resident.
  • Actively read the Pass down and MEMO book.
  • Provides supervision, coaching, and assistance to clients with their Activities of Daily Living (ADLs) such personal hygiene, dressing, shopping, meal preparation, and housekeeping
  • Monitors and ensure clients’ homes and living environments are safe and free of any hazards
  • Supports and coaches’ clients in maintaining food cleanliness and safety and promotes nutritious, portioned meals
  • Helps clients with laundry, including washing, drying, folding, and properly putting clothes
  • Accompanies and transports clients to medical appointments, community activities, and employment sites. Assist clients in building and maintaining social relationships.
  • Teaches and encourages life skills such as budgeting, cooking, respect for others, proper etiquette, and appropriate social interactions.
  • Implements person-centered support plans and de-escalates challenging behaviors to promote a positive environment
  • Ability to give simple clear instructions, patience with slow changes, the ability to communicate in a mode effectively with the individual’s learning needs and style.
  • Assists clients in medication usage and administration and monitors vitals
  • Reports all undesirable behavior, such as stealing, inappropriate sexual conduct, intimidation, disobedience and/or disrespect to the Supervisor and/or Executive Director.
  • May be required to provide cross coverage to other IDD service areas including but not limited to supported employment and adult day services
  • Follows training guidelines of regulatory agencies.
  • Documents all home training performed to include date, location, attendees, and subject content.
  • Reports unusual incidents through appropriate Risk Management, clinical and safety channels.
  • Attends regular training and staff meetings to ensure knowledge of changes being made.
  • The third shift is responsible for cleaning all communal areas.
  • Ensures clients’ rights are protected.
  • Is knowledgeable of and follows all safety procedures.
  • Implements Saint Francis Ministries policies and follows directives as required. Follows and adheres to all pertinent Saint Francis Ministries Standard Operating Procedures(SOP’s), rules, personnel policies, and procedures; related accreditation and licensure standards; and federal, state, and local rules, statutes, regulations, and contractual terms.
Qualifications
  • High School diploma (and/or GED),
  • First Aid/CPR.,
  • Mandt Training.
  • Prefer two years’ experience in related work areas.
  • Must be 21 years of age.
  • Must pass a drug screen, MVR, MS State Background Check and Child Abuse and Neglect Central Registry Clearance Check; pass a physical examination & TB Skin Test
  • Lifting Requirements of 50 lbs.
  • Valid Mississippi Driver’s License and Clear Driving Record.
