Note:  This position is funded by one or more grants.  As such, this position is not covered by the Trinity Health Severance Pay Plan.

REQUIREMENTS:   

  • Master's Degree in Social Services + 1 year of related social service experience OR Bachelor's Degree in Social Services + 3 years of related social service experience OR Associate Degree in Social Services + 4 years of related social service experience OR HS Diploma/GED +8 years of related social service experience. 

  • Valid Drivers’ License 

  • Ability to operate organizational vehicle/van 

  • Act 33/34/73 clearances 

  • Pre-employment Drug Test 

 

SCHEDULE: (hourly/non-exempt) 

 

37.5 hours per week primarily Monday-Friday 8 am-4pm with flexibility to meet consumer and/or program needs.  Some nontraditional hours (evenings, weekends, and holidays to meet the needs of the program.) 

 

ABOUT THE ROLE: The PATH Clinical Case Manager provides direct outreach and support to individuals experiencing homelessness throughout the community. This role helps both chronic and non-chronic homeless populations access and navigate Allegheny County's Continuum of Care services, including housing assistance, behavioral health services, substance use treatment, and public benefits programs. 

The position involves conducting outreach in a variety of settings, including shelters, encampments, community locations, and other agency sites. The Clinical Case Manager connects individuals with resources, advocates for their needs, and supports them in achieving greater stability and access to essential services.  

Accurate and timely documentation is required in both the county's HMIS system and Pittsburgh Mercy's Avatar reporting system. Success in this role requires flexibility, strong organizational skills, and the ability to manage multiple priorities while responding to the changing needs of persons served throughout the day. 

All duties are performed in accordance with applicable laws and regulations, as well as Pittsburgh Mercy's mission, policies, procedures, and standards of care.
