Heather Heights Senior Living is seeking an experienced and compassionate Licensed Practical Nurse (LPN) to join our Memory Care team on the day shift. We are looking for someone who understands the unique needs, behaviors, and challenges that can come with caring for individuals living with Alzheimer’s disease and other forms of dementia.
The right candidate will bring strong clinical skills along with the patience, empathy, and communication skills needed to help our residents feel safe, respected, and supported each day.
What You'll Do
- Provide compassionate nursing care to residents within our Memory Care community
- Administer medications and treatments in accordance with physician orders and established policies
- Monitor residents for changes in physical, cognitive, emotional, and behavioral status
- Recognize and appropriately respond to behaviors and symptoms associated with dementia and cognitive decline
- Communicate changes in resident condition to physicians, families, and appropriate members of the care team
- Collaborate closely with Personal Care Aides, Medication Technicians, and other team members to ensure consistent, resident-centered care
- Assist with care planning, documentation, and ongoing assessment of resident needs
- Provide guidance and support to direct-care staff, particularly when navigating challenging memory care behaviors
- Help create a calm, structured, and reassuring environment for our residents
- Build meaningful relationships with residents and their families
What We're Looking For
- Current New York State LPN license in good standing
- Previous memory care, dementia care, or Alzheimer's care experience strongly preferred
- Strong understanding of dementia-related behaviors and person-centered approaches to care
- Excellent observation, communication, and clinical judgment skills
- A calm and patient approach, especially in challenging or changing situations
- Dependable, compassionate, and committed to providing high-quality senior care
- Ability to work collaboratively as part of an interdisciplinary team

Day Shift: 7:00 AM – 3:00 PM
