Home Health Registered Nurse (RN)

John Knox Village Home Health is seeking a compassionate, skilled, and dedicated Registered Nurse (RN) to join our growing team.

This rewarding position provides one-on-one nursing care to patients in their homes, helping them achieve the highest possible level of health, independence, and quality of life. Unlike traditional hospital settings, Home Health Nurses enjoy greater autonomy while serving as a clinician, educator, advocate, and care coordinator. Through comprehensive assessments, clinical expertise, and meaningful patient relationships, the Home Health RN plays a vital role in improving outcomes for the elders and guests we serve.

Position Summary

The Home Health Registered Nurse (RN) is responsible for providing skilled nursing care to patients in their homes while coordinating services with physicians, caregivers, and interdisciplinary team members. The RN performs comprehensive assessments, develops and manages individualized care plans, monitors clinical status, provides patient and caregiver education, and ensures continuity of care. This position is essential to supporting positive patient outcomes and maintaining the highest standards of clinical excellence.

Essential Responsibilities

  • Conduct comprehensive physical, emotional, and nursing assessments, including monitoring vital signs and overall clinical status.
  • Develop, implement, and evaluate individualized care plans in collaboration with physicians and interdisciplinary team members.
  • Provide skilled nursing care, including medication administration, injections, IV therapy, wound care, and other physician-ordered treatments.
  • Monitor and document patient progress, response to treatment, and changes in condition.
  • Review lab results, diagnostic reports, and physician orders, reporting significant findings as appropriate.
  • Assess, manage, and monitor pain levels, implementing interventions to promote patient comfort and well-being.
  • Evaluate skin integrity, bowel and bladder function, nutritional status, hydration, mobility, and medication effectiveness.
  • Coordinate care among physicians, therapists, social workers, caregivers, and other healthcare professionals.
  • Provide education to patients and caregivers regarding disease management, medications, treatment plans, and home safety.
  • Ensure that all clinical documentation and medical records are complete, accurate, and compliant with regulatory standards.
  • Supervise and provide clinical oversight for Licensed Practical Nurses (LPNs) and Home Health Aides (HHAs) involved in patient care.
  • Visit patients regularly to assess care needs and verify that established care plans are being followed effectively.
  • Report significant changes in patient condition to the physician and Home Health Administrator.
  • Investigate and address patient and family concerns with professionalism and compassion.
  • Maintain accountability for medication management, including controlled substances, while reporting discrepancies according to policy.
  • Participate in Quality Assurance and Performance Improvement (QAPI) initiatives and clinical audits.
  • Support compliance with federal, state, and regulatory standards while assisting with implementation of new clinical requirements and best practices.
  • Practice in accordance with the Florida Nurse Practice Act and established professional nursing standards.
  • Maintain all required licenses, certifications, and continuing education requirements.

Qualifications

Education & Experience

  • Current Registered Nurse (RN) license in the State of Florida.
  • Current CPR certification required.
  • Minimum of one (1) to two (2) years of nursing experience preferred.
  • Experience in Home Health, Long-Term Care, Skilled Nursing, Rehabilitation, or Geriatric Care preferred.
  • Valid driver's license and reliable transportation required.

  • Proficiency with electronic health records (EHRs), Microsoft Office applications, and related healthcare technology.
  • Excellent communication and interpersonal skills.
  • Ability to effectively educate patients and caregivers.
  • Strong organizational, documentation, and time-management abilities.
  • Ability to work independently while managing multiple patient visits and priorities.
  • Commitment to maintaining patient confidentiality and providing culturally sensitive care.

Physical Requirements

  • Ability to stand, walk, bend, stretch, squat, and remain active throughout the workday.
  • Ability to lift, push, pull, and move up to 50 pounds.
  • Sufficient strength and dexterity to assist patients and operate medical equipment safely.
  • Ability to travel between patient homes and work in various home care environments.
