Job DetailsJob Location: 1700 EDUCATION AVE - PUNTA GORDA, FL 33950Position Type: Full TimeEducation Level: 4 Year DegreeTravel Percentage: NegligibleJob Category: Nonprofit - Social Services Makes clinical determination of mental status, suicidality, homicidality, or self-neglect of persons who present to the CRF or RC Responsible for screening, assessment, and discharge planning for persons receiving services at the CRF or RC Conducts individual and/or family sessions as well as leading group therapeutic activities for adults and children receiving inpatient services Obtains collateral information from persons who are familiar with the client in order to assess the level of risk Refers to licensed provider for involuntary assessment as needed. Assists patients with setting up outpatient appointments or with referrals to external community resources Assists with completion of emergency screenings for services as needed Provides information regarding mental health and substance use diagnosis and treatments Provides crisis de-escalation assistance to individuals in inpatient services Performs transfer evaluations per Baker Act law as needed Completes up-to-date documentation in accordance with the internal and external audit standards.
