The coordinator uses data analysis, structured improvement methods, workflow development, staff education, patient outreach, and cross-department collaboration to move improvement priorities from planning through sustained implementation.   This is a general Quality Improvement position with a primary specialization in the Aledade program. The coordinator serves as the day-to-day resource for assigned Aledade dashboards, quality measures, worklists, outreach workflows, data validation, performance monitoring, meetings, and issue escalation. Other projects may be assigned based on the Quality Improvement Plan and organizational priorities. ESSENTIAL DUTIES AND RESPONSIBILITIES Quality Improvement and Project Coordination Coordinate improvement initiatives aligned with the Quality Improvement Plan, strategic priorities, financially incentivized measures, grant requirements, and other assigned projects. Develop workplans, track milestones and assigned actions, maintain organized project records, address barriers with project partners, and escalate significant risks or delays. Collaborate with clinical, operational, and administrative teams to define problems, conduct root-cause analysis, test solutions, improve workflows, and monitor results. Develop and maintain procedures, workflows, job aids, training materials, and staff communications that support consistent implementation and sustained improvement. Data, Reporting, and Performance Improvement Collect, validate, analyze, and interpret clinical and operational data to identify trends, care gaps, outliers, and opportunities for improvement. Prepare accurate and timely reports, presentations, summaries, and action lists for internal and external stakeholders. Validate information across the electronic health record, Aledade, and other approved systems; document known data limitations or timing differences and escalate unresolved discrepancies. Aledade and Value-Based Care Serve as the primary day-to-day coordinator and internal resource for assigned Aledade tools, workflows, measures, training, meetings, and support needs. Review Aledade dashboards, HEDIS and other quality-measure specifications, care-gap reports, prioritized worklists, and high-risk or high-acuity patient lists. Monitor performance against established targets and identify areas requiring focused intervention. Maintain the Aledade program calendar, measure and report dictionary, workflow documentation, contact list, issue log, and current action plan. Ensure critical processes have documented primary and backup owners. Coordinate care-gap and patient-outreach activities by identifying patients for follow-up, distributing work, monitoring completion, and confirming required documentation is entered in the appropriate system. Within approved protocols, contact patients to obtain non-diagnostic information, provide basic education or self-management support, respond professionally to concerns, and connect patients with appropriate clinical or supportive services. Collaborate with clinical teams regarding care coordination, referral follow-up, patient concerns, and treatment-plan follow-up. Escalate clinical questions and do not independently diagnose, prescribe, or modify treatment plans. Prepare for Aledade performance and support meetings; document decisions, owners, due dates, and follow-up actions; and translate program guidance into clear workflows, staff education, and leadership updates. Participate in meetings and ongoing education, travel between clinic locations as needed, maintain accurate and confidential documentation, and perform other related duties as assigned. QUALIFICATIONS Education and Experience Three or more years of quality improvement or healthcare-related experience preferred. Associate or bachelor’s degree in healthcare, public health, health administration, or a related field preferred. A relevant combination of education and experience may be considered. Familiarity with HEDIS/UDS measures, specifications, and reporting requirements. Experience with Aledade or similar value-based care, accountable care, population health, or quality platform preferred. Experience using an electronic health record or comparable clinical information system. Knowledge, Skills, and Abilities Proficiency in data analysis, Microsoft Excel, and quality improvement methodologies. Knowledge of Medicare, Blue Shield, Anthem payer requirements, and quality improvement metrics. Ability to prepare clear reports, charts, presentations, procedures, workflows, and training materials. Excellent written and verbal communication, presentation, interpersonal, and patient-centered service skills. Strong attention to detail, organization, follow-through, and problem-solving skills. Ability to manage multiple projects and deadlines while working independently and collaboratively in a fast-paced environment. Proficiency with Microsoft Outlook, Word, PowerPoint, internet browsers, and virtual meeting tools, with the ability to learn new systems and complete required program training. Ability to travel between Hill Country Community Clinic locations as required. PERFORMANCE AND COMPLIANCE EXPECTATIONS   Produce accurate, timely, and reproducible reports, worklists, documentation, meeting materials, and follow-up actions. Maintain clear owners, workflows, milestones, risks, and current action plans for assigned measures and projects. Investigate, document, and escalate data discrepancies, barriers, delays, or errors through the appropriate path without avoidable delay. Follow approved patient-outreach workflows, privacy requirements, documentation standards, clinical escalation boundaries, and applicable laws and organizational policies. Protect the confidentiality of patient, employee, and organizational information. Demonstrate professionalism, flexibility, respect, accountability, responsible use of organizational resources, and reliable follow-through. WORK ENVIRONMENT AND PHYSICAL DEMANDS Work is primarily performed in indoor office and health clinic settings near normal office activity, medical equipment, computers, telephones, and printers. Noise levels are generally moderate.   The employee regularly communicates and listens, frequently uses hands to operate a computer and handle objects, and may sit, stand, walk, reach, bend, kneel, crouch, climb, or balance. The employee regularly lifts or moves up to 10 pounds and occasionally up to 25 pounds.
