Client Care Coordinator

Comprehensive Home Health Solutions (CHHS) – Reno, Nevada

Comprehensive Home Health Solutions is seeking an experienced, compassionate, highly organized Client Care Coordinator to join our Home Care team.

This is not a traditional desk-based care coordinator position. The Client Care Coordinator plays an active role in ensuring that our clients receive consistent, high-quality care while supporting and developing the caregivers who provide that care.

The ideal candidate enjoys working with older adults and families, is comfortable coaching and training caregivers, can manage multiple priorities, and is willing to step in when needed to make sure our clients are cared for.


What You’ll Do

As a Client Care Coordinator, you will:

  • Develop, maintain, and update individualized client care plans.
  • Build strong relationships with clients and families and serve as a resource for questions, concerns, and changing care needs.
  • Coordinate closely with caregivers, Scheduling, Intake, and the Lead Client Care Coordinator to ensure continuity of care.
  • Conduct required in-home caregiver supervisory visits and regular client/caregiver check-ins.
  • Monitor caregiver performance, professionalism, safety, and adherence to client care plans.
  • Participate in the onboarding, orientation, training, coaching, and mentoring of caregivers.
  • Provide hands-on field training and assist with caregiver competency validation.
  • Identify caregiver training needs and provide or coordinate additional education when needed.
  • Respond promptly to client and caregiver concerns and escalate significant issues appropriately.
  • Participate in daily morning huddles when not engaged in a client visit or other time-sensitive client responsibility.
  • Assist with admissions, staffing transitions, schedule changes, and other operational needs.
  • Maintain accurate and timely care coordination, supervisory, and training documentation.

An Important Part of This Role

Although this is primarily a Client Care Coordinator position, home care sometimes presents unexpected staffing challenges.

The Client Care Coordinator is expected to be flexible and, on an as-needed basis, help provide direct caregiving coverage for last-minute call-outs, unexpected vacancies, or other urgent unfilled shifts when CHHS needs assistance maintaining continuity of care.

This is not a position in which a predetermined percentage of your schedule will be spent caregiving. Rather, we are looking for someone who understands that when an urgent situation arises, taking care of the client comes first and the team works together to find a solution.

Schedule

The regular schedule is generally Sunday through Thursday.

Because healthcare and home care do not always operate on a predictable schedule, this position requires flexibility based on client needs, staffing situations, supervisory visits, admissions, and other operational requirements.

Who We’re Looking For

We are looking for someone who is:

  • Experienced in home care, caregiving, healthcare coordination, senior care, or a related field.
  • An excellent communicator who is comfortable working with clients, families, caregivers, and leadership.
  • Highly organized and able to follow through on multiple responsibilities.
  • Comfortable providing caregiver coaching, training, orientation, and mentorship
  • Able to identify a problem, communicate it promptly, and help develop a solution.
  • Compassionate with clients while also able to establish appropriate expectations and accountability with caregivers.
  • Comfortable working independently and as part of a fast-moving team.
  • Flexible and willing to help when unexpected client-care or staffing needs arise.
  • Able to travel to client homes throughout the service area.
  • In possession of a valid driver’s license and reliable transportation.
Preferred Qualifications

  • CNA, HHA, Medical Assistant, or another healthcare-related credential is preferred but not required for the right candidate.
  • Experience with older adults, dementia, chronic illness, complex care needs, caregiver supervision, or home care operations is strongly preferred.
