Overview of the Position:Â
The MDS Coordinator assumes full time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.
Job Functions:
- Provide quality, resident-centered care while protecting resident rights and confidentiality.
- Follow facility policies, HIPAA, and all applicable regulations.
- Maintain knowledge of long-term care practices, admissions, reimbursement, and MDS/RAI processes.
- Communicate effectively with residents, families, staff, and regulatory agencies.
- Follow safety, infection control, and emergency procedures.
- Prioritize tasks, maintain professionalism, and complete assigned duties.
- Perform other duties as assigned.
QUALIFICATIONS/REQUIREMENTS:
- Education:Â Graduate of an accredited school of nursingÂ
- License:Â Current active license as a Registered Nurse or Licensed Vocational Nurse (LVN)
Work Experience: At least 6 months to 1 year of MDS experience - SNF experience preferred
