Northern Illinois Hospice is seeking a Clinical Team Lead - CNA & Off Shift Operations to help guide our hospice team, support staff, and ensure exceptional patient care throughout evenings, weekends, and off-shift hours.

As the Clinical Team Lead - CNA & Off Shift Operations, no two days are the same. You’ll coach CNAs, support off-shift clinical staff, coordinate admissions and urgent referrals, solve complex patient care challenges, and ensure seamless hospice operations after hours and on weekends. You’ll be a trusted resource, mentor, and leader who helps deliver outstanding hospice care when patients and families need it most.


As the Clinical Team Lead - CNA & Off Shift Operations, you will:

  • Lead, coach, and support CNAs and off-shift clinical staff
  • Oversee staffing, scheduling, and coverage for hospice operations
  • Coordinate admissions, hospital discharges, and urgent hospice referrals
  • Serve as a clinical resource for symptom management and complex care situations
  • Conduct chart reviews, quality audits, and joint visits
  • Ensure compliance with regulatory, quality, and hospice standards
  • Support recruitment, onboarding, competency validation, and performance management
  • Partner with interdisciplinary teams to improve outcomes and patient experiences
  • Participates in clinical administrative on-call (AOC) rotation and serves as backup support as assigned.

What You Bring:

  • Active Illinois Registered Nurse license
  • Graduate of an accredited nursing program; BSN preferred
  • Minimum 5 years of nursing experience
  • Hospice or home health experience preferred
  • CHPN certification preferred (or obtained within two years)
  • CPR certification 
  • Valid driver’s license, insurance, and reliable transportation

Schedule & Compensation

Position hours are Monday through Friday, 7:00am to 2:00pm, however, hours may change based on business needs. This position is salary exempt, which requires coordination efforts prior to the workday and completion of work prior to departure. Scheduled clinical administrative on call (AOC) rotation includes assigned weekdays and weekends. 


Salary Description

The expected base salary range for this role is $ 91,600 - $132,732. However, final base pay will be carefully determined based on several factors, including education, experience, knowledge, skills, and abilities, as well as internal equity and alignment with market data.
