We are seeking an organized, empathetic, and detail-oriented Office Clerk and Intake Assistant to join our home health care team. In this dual role, you will serve as the primary point of contact for prospective patients, family members, physicians, and referral sources, while coordinating administrative workflows for our office. The ideal candidate excels in customer service, manages confidential information with discretion, and maintains smooth day-to-day operations in a fast-paced environment.

Office Clerk & Intake & Referral Management
- Answer phones, route calls, and handle incoming emails and faxes in a timely manner.
- Manage, sort, and distribute physical and electronic paperwork to designated staff or departments.
- Maintain organized physical and electronic medical/office records in compliance with agency policies and privacy regulations.
- Perform routine clerical duties including copying, scanning, faxing, and uploading/downloading documentation into the office portal.
- Handle incoming patient referrals, intake requests, and inquiries from hospital discharge planners, physicians, and families.
- Gather demographic, clinical, and insurance details to initiate service intake accurately.
- Contact patients, family members, and medical providers to collect missing information and confirm intake details.
- Assist in routing intake records to the appropriate clinical management team for authorization and scheduling.

- Qualifications
- High school diploma or equivalent.
- Prior experience in an office, clerical, or medical receptionist role (home health or healthcare setting preferred).
- Strong verbal and written communication skills with exceptional phone etiquette and customer service instincts.
- Proficiency with standard office software, basic data entry, and digital file management (faxing, scanning, uploading).
- High organizational skill with strict attention to detail and ability to multi-task under deadlines.
- High school diploma or equivalent.
