Job DetailsJob Location: Campus of Grace - San Antonio, TX 78217Position Type: Full TimeEducation Level: 4 Year DegreeSalary Range: $20.28 HourlyGeneral Role and Function: Responsible for completing assessments of clients’ eligibility for the Meal on Wheels San Antonio home delivered meal services based on internal and governmental standards. Identify and make appropriate referrals to internal support services to meet clients’ additional needs. Works closely with other coordinators to coordinate assessments, documentation, and files as needed. Works in conjunction with the Client Services team. Key Tasks and Responsibilities: Assess and re-assess clients for program eligibility as dictated by the government standards and organization policy. Use own vehicle to travel to the applicants’ and clients’ homes to complete assessments. Maintain all aspects of the client’s files; completing assessment and related paperwork, follow-up on required documents or information needed and making updates to clients’ files in a timely manner. Schedule appointments by prioritizing clients based on need and date of referral, completing an adequate number of assessments within the designated time, and keeping a daily log of home visits and mileage. Refer clients to other Meals on Wheels San Antonio programs when appropriate and giving clients the opportunity to make a financial contribution towards the cost of their meals as required by OAA. Communicate with clients and family/support systems on any issues that may arise with the clients’ condition, program status or referral(s) made within organization confidentiality policies. Coordinate with coworkers to ensure all assessments are completed in a timely manner. Share duties with other coordinators such as client filing and data entry, as needed. Appropriately screen incoming referral calls for program eligibility and make referrals on the clients’ behalf. Achieve and maintain a minimum of three (3) percent client-based donations. Assist with meal delivery when needed. Other duties as assigned by management.  Qualifications and Education Requirements: Bachelor’s degree preferred. Bilingual (English/Spanish) preferred but not required. Good communication skills in verbal and written. Good time management and map reading skills. Excellent office administration skills with advanced knowledge of Microsoft Office programs.  Must have reliable, personal vehicle and a valid Driver’s License, good driving record, and applicable insurance coverage required by the company policy. Team player with the ability to work independently. Physical requirements: Have mobility to work in a standard office setting and to use standard office equipment, including a computer; vision to read printed materials and a computer screen; and hearing and speech to communicate in person and over the telephone. Ability to work in a highly stressful environment dealing with a wide variety of challenges, deadlines, and diverse employee population. Position requires travel to client’s homes and will be subjected to changing temperatures and possible hazardous living conditions such as poor housing and vermin. Be able to move freely in and out of vehicle and climb stairs as needed to deliver meals and/or complete client assessments. Able to occasionally lift up-to twenty-five (25) pounds to move office supplies or other equipment. Qualifications
