Under the Native Connections grant program funded by the Substance Abuse and Mental Health Services Administration (SAMHSA), the Community Outreach Coordinator will function as an integral member of the healthcare team at SDCHC. The Native Connections program aims to support our mission of fostering resilience and reducing the impact of substance abuse and mental health issues on American Indian/Alaska Native (AI/AN) youth.
The 4-in-1 Program is funded by Indian Health Service (IHS). The 4-in-1 Program aims to ensure comprehensive, culturally acceptable personal and public health services are available and accessible to Urban Indians. The Program Manager will be tasked with leading SDCHC’s efforts to improve quality measures in 1. health promotion and disease prevention, 2. immunization, 3. alcohol- and substance use-related, and 4. mental health services.
The Outreach Coordinator will be tasked with establishing and integrating a community outreach approach targeting prevention and early intervention services while decreasing suicidal behaviors and substance use among AI/AN youth. The Outreach Coordinator will also be tasked with health promotion, disease prevention and immunization services for Urban Indians.
The Community Outreach Coordinator will work closely with the Behavioral Health Department, the Native Connections Program Manager and the 4 in 1 Program Manager, the Data Analytics Specialist, and the Grants Team. The Community Outreach Coordinator will be responsible for the implementation of any community engagement activities required by the Native Connections and 4 in 1 grant, while meeting all established grant deadlines and working collaboratively across departments. The Community Outreach Coordinator will engage directly with AI/AN youth, the larger community, and other local community agencies.
The Community Outreach Coordinator will assist in the development of a Community Needs Assessment, Community Readiness Assessment, and Tribal Strategic Action Plan. The Community Outreach Coordinator will be responsible for developing community outreach plans, events, and materials, including social media campaigns and messaging. The Community Outreach Coordinator will additionally spearhead the development of an Interagency Advisory Council and a Youth Advisory Board. The Community Outreach Coordinator will assist in data collection and reporting on any metrics related to community outreach.
The Community Outreach Coordinator will establish and maintain effective working relationships with medical staff, organizational leaders, and other departments and foster a collaborative environment with department leadership and staff to achieve program goals. The Community Outreach Coordinator will participate in work groups or committees as required for the success of the Native Connections program.

Primary Functions:

- Develop strong resources and engaging methods of delivery of healthcare messaging to Urban Native Americans, while understanding the sensitivity necessary in serving populations that may be affected by historical trauma.
- Conduct targeted outreach and training to community-based organizations.
- Participate in events/fairs that could potentially result in partnerships with San Diego Community Health Center.
- Maintain tracking logs of outreach efforts.
- Work Collaboratively to develop outreach strategies.
- Understanding, appreciation of, and strong personal commitment to the issues involved in diversity.
- Monitor receipt and accuracy of data submitted by nutrition and physical activity staff.
- Create and manage regular digital communication performance reports and key takeaways, including key analytics for the purpose of maximizing enrollment.
- Facilitate presentations in the community.
- Working evenings and weekends may be required for special events/meetings.
- Other duties as assigned.

Minimum Qualifications:

- High school diploma or equivalent required
- Bachelor’s degree in marketing/healthcare and or related field.

- Experience serving a multicultural population.
- FQHC and/or public health background.
- Familiarity with Community Health Clinics and/or Indian Health Clinics
- Experience working in the substance abuse prevention and mental health fields.

Special Conditions of Employment:
- CPR/BLS Certification: Maintain a current Basic Life Support (BLS) certification issued by the American Heart Association (AHA), the American Red Cross, or an equivalent organization. Certification must include an in-person, hands-on skills assessment; online-only certifications are not accepted.
- Annual Background Checks: Consent to annual background checks as a condition of continued employment, to ensure compliance with organizational standards and eligibility requirements.
- For-Cause Drug Screening: Comply with drug screening requirements when initiated by the organization for cause, to support a safe, compliant, and drug-free workplace.
- Ongoing Compliance Requirements: Maintain up-to-date compliance with all required annual renewals, including certifications, physical examinations, TB testing, and mandatory regulatory trainings as assigned by SDCHC, including HIPAA and 42 CFR Part 2 confidentiality training.
- This position is funded in whole or in part by federal grant funds. Continued employment in this position is contingent upon the availability of grant funding, and this position will end upon the expiration, termination, or non-renewal of grant funding, unless alternative funding is identified and approved by SDCHC.
