Senior Licensed Vocational Nurse (LVN) – Primary Care

UCSF's Temporary Employment Program (TEP) recruits and hires temporary employees for immediate clerical and technical support services to UCSF Departments and various off-campus locations. UCSF departments deploy temporary employees to work on special projects, fill in for regular employees who are on vacation or leave, or to temporarily fill a vacant position during recruitment. Frequently temporary employees become successful candidates for career and limited appointment positions.

Employment Duration: 3 months (potential to extend)

Under the supervision of the UCSF Primary Care Administrative Nurse Manager the Licensed Vocational Nurse utilizes the nursing process to assess and identify the individual needs of patients through observation of disease symptoms, emotional state, laboratory reports, cultural and socio economic background and response to medical regimen and treatment.  Additionally, s/he is required to learn and apply the Patient Centered Medical Home Standards and Ten Building Blocks of Primary Care.He/she is responsible for providing nursing care to patients in the ambulatory setting. The LVN will work as an important member of the care team, working closely with the RN and the physicians to deliver all components of nursing care will be individualized to meet the patient’s physiological and psychosocial developmental level and needs. The LVN will be responsible for chart abstraction.   The Licensed Vocational Nurse consistently projects a professional and courteous demeanor and shall comply with the Medical Center House and Telephone Standards. In addition,will have clinical responsibility for administration of medications/vaccines, lab result review and monitoring, treatments and procedures pertinent to pediatric & adult primary care such as ear irrigations, wound care and urethral catheterization, as well as patient education. The LVN must have flexibility to cover multiple UCSF medical center primary care clinic sites.

This individual will utilize his/her good customer service and communication skills on a daily basis via phone/electronic contact, mail correspondence, and personal contact with both internal and external customers.  This employee will utilize computer skills in ApeX and other UCSF computerized programs.  This individual supports the standards of the policies and procedures of the practice, and is responsible for maintaining current knowledge of pharmacology and competency in modes of administration, and complies with all Ambulatory Services and Medical Center policies and procedures for infection control, safety, and administrative and clinical practice.  S/he is required to adhere to the Telephone and House Standards as adopted by the Medical Center, as well as demonstrating UCSF’s PRIDE values and the AIDET fundamentals of communication in all patient care and stakeholder interactions.

• Identify patient needs and responses to treatment through observation of symptoms, emotional state, and cultural or socioeconomic background
• Recognize and report physical and emotional conditions or concerns to the RN or physician
• Alert providers to unusual observations or acute patient symptoms
• Collaborate with the health care team to develop care plans and patient goals
• Provide individualized nursing care based on patient growth, development, and treatment needs
• Deliver age-appropriate care across neonatal, pediatric, and adolescent populations
• Perform patient intake, maintain exam room cleanliness, and ensure patient privacy
• Assist patients with limited mobility
• Prepare and educate patients on diagnostic and therapeutic procedures
• Assist with procedures and treatments as needed
• Maintain and safely use clinical equipment
• Document patient care, responses, and treatments accurately and legibly
• Administer medications and maintain current pharmacology knowledge
• Respond appropriately to emergencies, including CPR and safety protocols
• Comply with infection control, safety, and regulatory standards
• Maintain communication with patients, families, and care teams through various channels
• Utilize clinical systems, including APeX, to support patient care
• Maintain accurate patient records and documentation
• Demonstrate professional communication and cultural sensitivity
• Coordinate retrieval and documentation of external medical records
• Identify and support patient education needs
• Provide information to support informed patient decision-making and appropriate referrals
• Reinforce health teaching using understandable language and verify comprehension
• Participate in performance improvement activities and data collection

Required Qualifications

• Licensed Vocational Nurse in the State of California

• Six months to 2 years of experience as a Licensed Vocational Nurse in an ambulatory setting

• Current CPR Certification (BLS)
• Demonstrated verbal and written communication skills
• Strong customer service orientation

• Knowledge and abilities essential to the successful performance of the duties assigned to the position, including demonstrated excellent communication skills, problem-solving ability, and high comfort level with learning new technology/systems.

• Basic computer/keyboarding skills.

• Prior experience with electronic systems/records. 

• Demonstrated ability to work in a fast-paced environment.

• Trained and licensed to perform phlebotomy 
   

Preferred Qualifications

• Recent experience working in pediatrics strongly preferred
• Primary care experience strongly preferred 
• Proficiency in EPIC strongly preferred
