Position Overview:

The MDS Coordinator assumes full-time administrative authority, responsibility and accountability to coordinate the completion of the required minimum data set and with other members of the inter-disciplinary team, develops and implements a plan of care that meets the individual needs of each resident.


Resident Assessment & Care Planning:

  • Conduct resident assessments as required.

  • Develop, evaluate, and update care plans in compliance with regulations. 

  • Complete MDS (Minimum Data Set) within required timeframes. 

  • Ensure all care needs are included in the resident’s plan of care. 

Compliance & Documentation:

  • Ensure accurate documentation of assessments, interventions, and responses.

  • Maintain electronic MDS records and comply with CMS (Centers for Medicare and Medicaid Services) guidelines.

  • Review and validate MDS reports to ensure accuracy and compliance.

Interdisciplinary Team Collaboration:

  • Work closely with nursing staff, rehab therapists, and other team members.

  • Participate in care plan meetings and quality assurance initiatives.

  • Communicate policies and procedures effectively to the nursing team.

Regulatory & Policy Compliance:

  • Maintain knowledge of county, state, and federal nursing regulations.

  • Ensure all MDS assessments and documentation align with compliance requirements.

  • Stay updated on industry regulations and advancements in geriatric nursing.

Resident Rights & Advocacy:

  • Demonstrate respect for residents' rights, dignity, and confidentiality.

  • Ensure resident care aligns with professional nursing standards.

Reimbursement & Billing Coordination:

  • Understand and coordinate MDS assessments with reimbursement processes.

  • Work with the Business Office Manager on Medicare, Medicaid, and HMO billing.

Training & Staff Development:

  • Attend and apply continuing education to maintain nursing competency.

  • Provide guidance and training to nursing staff on MDS processes and compliance.

Emergency Preparedness & Safety:

  • Maintain knowledge of emergency and disaster procedures.

  • Ensure resident and staff safety in emergency situations.

  • Comply with infection control and workplace safety policies.



Qualifications:

  • Graduate of an accredited nursing school.

  • Active and valid Registered Nurse (RN) license in the state of California.

  • Minimum one year of experience in long-term geriatric nursing care or SNF.

  • Experience with Minimum Data Set (MDS) assessments is preferred.

  • Must have exceptional communication and customer service skills to interact effectively with residents, families, staff, and external agencies.

  • Must be able to handle stressful situations in a skilled nursing environment, including resident care emergencies, family concerns, state inspections, and interactions with medical professionals.

  • Must comply with HIPAA regulations to protect patient health information.

  • Must have an active BLS through the American Heart Association
