About the Program
The High Fidelity Wraparound (HFW) Program is an intensive, team-based care coordination model designed for children, youth, young adults, and families with complex behavioral health needs who are often involved with multiple systems such as mental health, child welfare, education, juvenile justice, and housing services. The goal is to help individuals remain successful in their homes, schools, and communities while reducing the need for higher levels of care. High Fidelity Wraparound is recognized as an evidence-based, family-centered approach that brings together multidisciplinary team members, natural supports, family members, and service providers to develop individualized plans that improve outcomes and strengthen support systems.
Productivity Expectation: This position has a baseline productivity expectation of 50%, generally equivalent to an average of approximately 3.5 hours of direct client service per workday. The meeting of such requirements will be addressed in accordance with the Company policies and applicable law.
About the Role
Clarvida is seeking a Psychiatric Nurse Practitioner to provide psychiatric evaluation, assessment, diagnosis, medication management, and ongoing treatment services for children, adolescents, young adults, and families enrolled in the High Fidelity Wraparound Program. Working under the supervision and collaboration of a Staff Psychiatrist, this role serves as an integral member of the multidisciplinary treatment team and helps ensure participants receive comprehensive, coordinated behavioral health care.
Responsibilities
· Conduct comprehensive psychiatric evaluations, assessments, and diagnostic reviews
· Obtain psychiatric, medical, developmental, and behavioral health histories
· Perform mental health assessments and formulate treatment recommendations
· Prescribe and manage psychiatric medications under appropriate collaborative practice guidelines
· Monitor medication effectiveness and evaluate side effects or adverse reactions
· Order and review laboratory studies and diagnostic testing as clinically indicated
· Obtain informed medication consents and provide medication education to clients and families
· Collaborate with psychiatrists, therapists, care coordinators, peer specialists, and other treatment team members
· Participate in treatment planning meetings and interdisciplinary case consultations
· Maintain complete and accurate clinical documentation within the electronic health record system
· Complete psychiatric assessments, medication reviews, treatment plans, and required program documentation
· Conduct home visits and community-based visits when clinically appropriate and necessary
· Participate in staff meetings, case presentations, and clinical trainings
· Support implementation of High Fidelity Wraparound and community-based treatment models
· May provide clinical supervision and consultation to LVNs, LPTs, and other assigned staff
· Maintain compliance with HIPAA, ethical standards, and organizational policies
· Perform additional duties as assigned to support program operations and client care
Required Qualifications
· Master of Science in Nursing (MSN) from an accredited college or university
· Current California Nurse Practitioner license in good standing
· Current furnishing number and DEA registration, or ability to obtain upon hire
· Board Certification as a Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) preferred
· Experience conducting psychiatric evaluations, medication management, and behavioral health assessments
· Knowledge of psychiatric disorders, psychopharmacology, and community behavioral health practices
· Strong clinical judgment, assessment, and documentation skills
· Excellent interpersonal, communication, and collaboration skills
· Ability to work effectively as part of a multidisciplinary treatment team
· Valid California driver's license, reliable transportation, and active automobile insurance
Preferred Qualifications
· Psychiatric Mental Health Nurse Practitioner (PMHNP-BC) certification
· Experience serving children, adolescents, young adults, and families
· Experience in community-based behavioral health, Wraparound, Full Service Partnership (FSP), ACT, or intensive outpatient programs
· Experience working with Medi-Cal populations and county behavioral health systems
· Bilingual Spanish preferred
· Experience conducting home-based or field-based psychiatric services
