Description

SUMMARY:  

The Medical Technician/Case Manager serves as a dual-role member of the interdisciplinary treatment team at Detox, providing both medically monitored support and case management services. This position assists with medication administration, vital signs, screenings and client monitoring while also coordinating discharge planning, community referrals, housing resources, and continuity of care. The goal of this position is to improve client outcomes by integrating medical support with early discharge planning throughout the client’s stay.

DUTIES AND RESPONSIBILITIES:


Medical Technician Responsibilities-


· Assist with medication administration according to provider orders. 

· Obtain and document vital signs, blood glucose monitoring, BACs, urine drug screenings. 

· Complete admissions, and discharges.

· Monitor withdrawal symptoms and promptly report changes in client condition to nursing staff.

· Complete medication counts and maintains medication documentation (MAR/EHR)

· Conduct routine safety rounds and wellness checks.

· Maintain inventory of medications and medical supplies.

· Document client interactions accurately within EHR. 


Case Management Responsibilities-


· Complete psychosocial and resource assessments upon admission.

· Begin discharge planning at admission and update plans as needed throughout treatment.

· Coordinate referrals to residential treatment, outpatient services, MAT providers, housing, transportation, employment resources, behavioral health providers, and community agencies. 

· Coordinate appointments with medical providers, behavioral health providers, probation/parole, CYFD, courts, and other community partners.

· Develop individualized after-care plans with each client.

· Collaborate with nursing, providers, clinicians, peer support, and admission to remove barriers to successful discharge. 

· Follow up on pending referrals and ensure continuity of care. 


Shared Responsibilities-


· Maintain HIPAA and 42 CFR Part 2 confidentiality.

· Participate in interdisciplinary treatment team meetings. 

· Participate in CARF, Licensing, audits, quality improvement activities, and surveys.

· Maintain professional boundaries and therapeutic relationships.

· Assist with crisis intervention and de-escalation. 

· Participate in ongoing training and competency assessments. 

· Perform other duties as assigned. 


MINIMUM QUALIFICATIONS:


Skills/Knowledge/Abilities:


1. One year of working in substance use treatment, behavioral health preferred.

2. Experience with electronic medical records. 

3. Experience coordinating community resources preferred. 

4. Medication administration and documentation.

5. Excellent communication and documentation skills. 

6. Crisis intervention and de-escalation.

7. Time management and ability to prioritize multiple responsibilities.

8. Strong organizations skills.

9. Ability to work independently while functioning as part of an interdisciplinary team.

 

Education and Experience:


1. High School Diploma or GED required. 

2. Certified Nursing Assistant (CNA), Certified Medical Assistant (CMA), EMT, or equivalent clinical certification required.

3. Case Management Certification preferred or willingness to obtain within one year. 

4. Current CPR/BLS Certification

5. Valid New Mexico Driver’s License
