Responsibilities:


Job Summary
Coordinate all intake activities to establish home care for patients. Schedules services as determined by physician orders, contact patient/family, referral source and appropriate staff to coordinate services. Manage the patient schedule to provide for timely and coordinated care for all patients. Triage inbound calls from patients, families, physicians and staff members to appropriate clinical or operational personnel ensuring timely, responsive care in the most cost-effective manner. Interfaces with multidisciplinary team members to provide optimum care. Utilize exceptional communication and interpersonal skills to enhance and encourage collaborative staff and community relationships. Acts as liaison to in-patient case managers and other referral sources in the care coordination of home health services.
Essential Functions
  • Receives orders for Home Health services from referral sources. Obtains necessary information to coordinate all services. Coordinates start of service with referral sources and patient. Creates and maintains patient & employee schedule to provide timely and coordinated care for all patients. Documents coordination in electronic medical record.
  • Serves as hospital liaison to provide in person care coordination for referral management. Will meet with patients, case management team & other health care services members to identify home care needs and provide care coordination to ensure a timely discharge and start of services for home health.
  • Develop relationships with referring sources with the emphasis on continuity of care within HonorHealth. Provide education to Case Managers on HonorHealth Home Health. Represents HonorHealth Home Health referring sources Interfaces with multidisciplinary team members to provide optimum care. Utilize exceptional communication and interpersonal skills to enhance and encourage collaborative staff and community relationships. Obtain benefits and eligibility for new patients as well as recertification’s utilizing all resources effectively and efficiently.

  • Associate's Degree - Preferred

Experience
  • 1 year experience in referral, pre-authorization or scheduling - Required
  • 1 year home health or post-acute liaison experience - Required
  • 1 year customer/patient facing experience - Required
  • 2 years Home Health or post-acute liaison experience - Preferred
Licenses and Certifications
  • Proof of current vehicle insurance - Required
  • Driver License Valid driver’s license - Required
