Job DetailsJob Location: Scranton, PA 18503 Identifies patient mental health problems and prescribes treatment. Implements mental health services utilizing therapeutic regimens. Provides mental health education to patients and their families. Provide health/behavioral health assessment (including differential diagnoses, prognosis), treatment recommendations and treatment planning for patients across program types. Provide medication management for patients on medication and intermittent reassessment (as needed) for those assessed (but not on medication). Provide counseling, brief therapy, psychoeducation about behavioral health and other care for patients as planned by the treatment team and to support the treatment plan of the client. Participate in case reviews and contribute to treatment planning goal setting. Provide consultations to non-medical staff on complex cases. Consult with primary care provider, substance abuse counselors, blended case managers, and other mental health professionals as needed. Maintain professional behavior and relationships with all Friendship House staff and referral resources. Attend staff meetings, provider meetings and other meeting as required. Fill out and maintain patient-related paperwork, including federal- and state-mandated forms. Participate in assessments and medication management off the main site as needed. Provide crisis intervention, emotional support, resources and referrals as needed. Comply with Agency policies and procedures, including those regarding conduct, confidentiality, and record keeping. Performs in-person and/or telehealth services to identified populations either at the remote location or in the office as directed by agency requirements and policies. Perform duties in a professional manner in response to clients, co-workers, and situations in which own values are challenged. Other duties as assigned. Qualifications Board Certified or Eligible in Nurse Practitioner Current licensure in Pennsylvania 2 years’ experience working in mental health setting.
