Responsibilities

  • Assess and triage incoming patient calls, evaluating cardiac symptoms such as chest pain, palpitations, shortness of breath, dizziness, and changes in blood pressure to determine urgency and appropriate next steps.
  • Provide telephone triage and clinical guidance, directing patients to the appropriate level of care, emergency services, or timely cardiology follow-up based on their symptoms and needs.
  • Coordinate urgent and emergent care for patients experiencing acute or concerning cardiac symptoms, working closely with cardiologists and the clinical team to ensure timely intervention.
  • Communicate effectively with cardiologists, providers, and interdisciplinary team members to facilitate care, address patient concerns, and support individualized treatment plans.
  • Educate patients and families on cardiac conditions, medications, treatments, diagnostic testing, lifestyle changes, and symptom management to help them actively participate in their heart health.
  • Provide ongoing support to patients managing chronic cardiovascular conditions, helping them understand their care plan and when to seek additional medical attention.
  • Accurately document patient calls, assessments, triage decisions, interventions, and follow-up plans in the electronic health record (EHR).
  • Monitor and follow up on patient needs, test results, referrals, and care plans to promote continuity of care and positive cardiac outcomes.

Qualifications

  • Registered Nurse (RN) – Active/unencumbered RN license in the state of practice or compact license including the state of practice; all levels of RN experience will be considered. Prior cardiology or specialty care experience is a plus, but not required.
  • ADN required; BSN preferred. For positions within UnityPoint Clinic, team members may be required to work at or float to other locations to support optimized staffing.
