Your Territory: St. Cloud & Sauk Centre
We're specifically looking for a PTA interested in serving patients throughout the St. Cloud and Sauk Centre areas and surrounding Central Minnesota communities.
Patient scheduling is coordinated geographically whenever possible to help cluster visits and create a more efficient field schedule.

And you'll provide treatment specifically designed to help patients function more safely within their everyday lives.

What You'll Do
Working under the direction of the supervising Physical Therapist, you'll:
- Provide one-on-one Physical Therapy treatments in patients' homes
- Carry out individualized plans of care
- Provide therapeutic exercise and strengthening
- Perform gait and mobility training
- Improve balance, coordination, transfers, and endurance
- Reinforce home exercise programs
- Educate patients and caregivers on safe mobility and therapeutic techniques
- Monitor patient response and tolerance to treatment
- Recognize and communicate changes in condition
- Update the supervising PT on patient progress
- Collaborate with nurses, OTs, SLPs, physicians, and other interdisciplinary clinicians
- Complete timely and accurate clinical documentation

This position offers $70,000–$102,000 in annualized earning potential through a paid-per-visit compensation model.

- Active Minnesota Physical Therapist Assistant (PTA) license
- Strong therapeutic exercise and functional mobility skills
- Excellent patient and caregiver communication
- Ability to work independently and manage a field-based schedule
- Strong collaboration with supervising Physical Therapists
- Ability to recognize and communicate changes in patient status
- Dependable and organized approach to patient care
- Valid driver's license and reliable transportation
- Ability to travel throughout the assigned St. Cloud & Sauk Centre territory
