Community Based Therapist

Position Summary:

Under the direct supervision of the Supervising Therapist, the Community-Based Therapist provides person-centered, recovery-focused mental health and substance use disorder (SUD) treatment to individuals and families in community settings, including clients' homes and other appropriate community locations. The position is designed to increase access to treatment for individuals who may benefit from receiving services outside of the traditional clinic setting.

The Community-Based Therapist may also provide services to clients within the Integrated Health Care Clinic and may carry a blended caseload of community-based and clinic-based clients with mental health and/or substance use disorder treatment needs. This position requires flexibility in location, scheduling, and service delivery to meet client, family, and program needs.

Essential Job Functions:

• Provide person-centered, recovery-focused individual, family, couples, and group psychotherapy for mental health and substance use disorders using evidence-based practices.
• Conduct comprehensive intake and ongoing clinical assessments; develop, monitor, and update individualized treatment plans with measurable goals and objectives.
• Provide crisis assessment and intervention while following established safety, response, and escalation procedures.
• Deliver flexible clinic- and community-based services, including in-home therapy, travel throughout the service area, and one evening per week until 7:00 p.m. as needed.
• Coordinate care and advocate for clients and families by collaborating with providers, schools, community organizations, and other support systems and facilitating appropriate referrals and resources.
• Complete timely and accurate EHR documentation; maintain confidentiality and comply with agency policies, regulatory requirements, professional standards, licensing, and quality expectations.
• Meet established productivity requirements, including 21 hours of direct clinical services per week after caseload build-up, while participating in supervision, meetings, training, and professional development.

Salary Range: $66,000

Qualifications:

• Master's degree in Social Work, Mental Health Counseling, Marriage and Family Therapy, or another qualifying mental health discipline.
• Current New York State license or permit to practice in an applicable discipline.
• One year of post-graduate clinical experience preferred; relevant prior experience may be considered in accordance with program requirements.
• Experience providing individual and family-focused mental health and/or SUD treatment; community-based behavioral health experience preferred.
• Ability to travel throughout the designated service area to provide services in clients' homes and other community settings.
• Ability to work a flexible schedule based on client and program needs, including one evening per week.
