Rn Supervisor for 132 Bed Facility Long Term Care, Skilled Nursing Facility.
BROAD RESPONSIBILITIES:
Involved in admissions and assessments, play a crucial role in patient care by gathering essential information to inform treatment plans, conduct thorough assessments, verifying patient information, and developing individualized care plans based on patients needs.
- Patient Assessment: Performs comprehensive evaluations, including physical assessments and health histories, to determine patient needs.
- Medication Reconciliation: Ensure accurate medication list are maintained, identifying any discrepancies.
- Care Plan Development: Accurate documentation of patient information and care plans is essential for continuity of care.
- Patient Education: Explain facility policies and procedures to residence and families, addressing any concerns.
HOURS: Monday - Friday 11am to 7pm or four 10 hour shifts.
