Through compassionate nursing support, thoughtful care coordination, and ongoing case management, you’ll help ensure patients receive timely access to the care, services, and resources they need. In this outpatient palliative care role, you’ll provide telephonic nursing support, patient triage, clinical care coordination, and ongoing case management while working closely with palliative care providers, patients, caregivers, primary care providers, specialists, and community partners. You’ll help address changing clinical needs, close gaps in care, and create a more coordinated and supportive healthcare experience for patients and their families. Key Responsibilities Provide telephonic nursing assessment, triage, education, and support to palliative care patients and caregivers. Coordinate clinical needs, including medication refills, durable medical equipment (DME), home health, home infusion, laboratory services, referrals, appointments, and other patient-care services. Monitor changes in patient condition and appropriately escalate urgent or emergent concerns to the appropriate provider. Support the development, implementation, and ongoing review of individualized plans of care in collaboration with the interdisciplinary team. Provide proactive clinical follow-up for patients enrolled in Principal Care Management (PCM), including care coordination, disease-specific support, medication management, patient/caregiver education, and communication with treating providers. ⏱️ Accurately document and track time spent performing qualifying PCM activities to support clinical continuity, regulatory compliance, and appropriate billing. Identify clinical and non-clinical barriers to care and coordinate resources necessary to address patient and caregiver needs. ‍⚕️ Support palliative care Nurse Practitioners by managing follow-up needs and care-coordination activities, allowing providers to focus on direct patient care. Participate in interdisciplinary team meetings, quality-improvement activities, and other program initiatives. Minimum Qualifications Current and valid Florida Registered Nurse (RN) license. Graduate of an accredited nursing program. ❤️ Current CPR certification. Strong clinical assessment and nursing judgment skills. Strong care coordination abilities. Excellent communication skills. Strong documentation and organizational skills. Experience in palliative care, hospice, geriatrics, chronic disease management, or care management is preferred. Qualifications Qualifications Education & Licensure Current, unrestricted Registered Nurse (RN) license in the State of Florida. Graduate of an accredited school of nursing with an Associate or Bachelor’s degree in Nursing. ❤️ Current CPR certification in accordance with organizational requirements. Valid driver’s license, reliable transportation, and automobile insurance meeting organizational and State of Florida requirements. Clinical Experience Minimum of two years of clinical nursing experience required. Experience in palliative care, hospice, geriatrics, chronic disease management, case management, home health, or care coordination is strongly preferred. Experience providing telephonic nursing assessment, patient triage, care coordination, and patient/caregiver education. Demonstrated ability to recognize changes in patient condition and appropriately escalate routine, urgent, and emergent clinical concerns. Knowledge of palliative care principles, chronic illness management, symptom management, and goals-of-care support. Experience coordinating patient-care services such as durable medical equipment (DME), home health, medications and refills, laboratory services, referrals, specialist appointments, and community-based healthcare resources. Care Management & Documentation Knowledge of or experience with Principal Care Management (PCM) or other care-management programs, including documentation of care-management activities and accurate tracking of qualifying clinical time, preferred. Strong clinical documentation skills with the ability to maintain accurate, timely, and compliant electronic health records. Proficiency with electronic health records (EHR), Microsoft Office, and other healthcare technology. ✨ Skills & Abilities Excellent verbal and written communication skills. Strong critical-thinking, problem-solving, organizational, and time-management skills. Ability to independently prioritize and manage a high-volume caseload while maintaining attention to detail and quality of care. Ability to collaborate effectively with Nurse Practitioners, physicians, social workers, caregivers, and other members of the interdisciplinary care team. Ability to work independently, exercise sound clinical judgment, and appropriately prioritize changing patient needs. ⭐ Preferred Certification Hospice and palliative care certification or another relevant specialty certification is preferred.
