Evaluates all patients referred by a physician to PT. Evaluation will be relevant to the diagnosis and will include but not be limited to: A. Specific area and extent of involvement. B. Limitation of motion. C. Loss of strength and/or coordination. D. History re: present diagnosis. E. Functional ability with application to activities of daily living. 2. Provides treatment, under direction of referring physician in the following areas: A. Increase range of motion. B. Increase strength. C. Increase coordination. D. Increase functional ability. E. Instructions and training in use of assistive equipment. F. Instruction and training in use and care of assistive ambulatory devices. G. Instruction to patient and his/her family in home treatment program. H. Instruction and training in proper body mechanics. I. Instruction and training in activities of daily living. 3. Maintains a daily time and mileage record for each visit patient is treated and submit in accordance with agency policy. 4. Schedules patients for treatment as needed. 5. Maintains evaluations and progress notes on each patient according to VNA policies and procedures and submits in accordance with agency policy. 6. Maintains an awareness of other therapies the patient may be receiving and confers with these disciplines as needed. 7. Maintains communication with VNA staff regarding patient’s treatment progress. 8. Maintains communication, verbal and written, with physician regarding patient’s status in PT. 9. Remains aware of changes in health care fields and implements practice changes as needed. 10. Provides in-service education programs for agency staff as needed. 11. Orders and maintains supplies and equipment as needed. 12. Remains current in the practice of PT through attendance at conferences, continuing education programs, in services, and other meetings to increase and update therapist’s knowledge in the PT field. Submits documentation to be filed in human resource record. 13. Attends weekly case conferences. 14. Participates in departmental record reviews and other QAPI activities. 15. Supervises/conferences with PTA each day they complete a visit. 16. Performs functional evaluations of patients progress every 30 days or as mandated by payor source. 17. Is knowledgeable regarding agency policy, procedures, and general information. A. Attends selected workshops, staff meetings, and seminars to enhance job performance & knowledge of services. B. Reports to work at the scheduled time and uses time efficiently. C. Willingly performs other duties outside of job responsibilities as may be required. QualificationsEducation, Training & Experience 1. Must be a graduate from an accredited School of Physical Therapy with a Bachelor’s degree in Physical Therapy. 2. Must be licensed in the State of Indiana. 3. Two years’ experience required. 4. Geriatric or home health experience desirable. 5. Must meet requirements for employment, including but not limited to, possession of a valid driver’s license and vehicle insurance, successful completion of a criminal background report, post offer physical exam and drug screen. Tools & Equipment General office equipment: office telephone, computer, cellular phone, periodical, medical equipment needed to provide PT services.   Standards of Service Excellence Attitude ▪Promptly responds to customers in a friendly manner.
