Job Summary:

In the home visiting position of the Prenatal Care Educator/Community Health Worker, you work to improve overall health and birth outcomes with clients on your caseload. 

You deliver health education to clients, develop pathways to better health or health improvement plans, and connect clients to vital resources.

REQUIREMENTS 

  • Education: High School Diploma or equivalent 
  • Skills: Strong time management, computer, and organizational skills. Must be able to understand and follow directions, communicate clearly in writing and orally, and enter computer data with a high accuracy rate.Must have the ability to advocate for those from underserved populations. Must be able to respect the diverse cultures and beliefs of clients and understand the issues that women and minority populations face related to health disparities, specifically infant mortality. Must have a passion for helping others and desire ongoing professional development. 
  • License: Must have reliable transportation and be able to travel locally regularly. Must have current car insurance and a valid driver’s license. 
  • Certification: CPR, BLS  

PREFERRED REQUIREMENTS 

  • Education: Associate’s degree 
  • Years of Experience:One or more years of community outreach-related experience. 
  • Certification:Community Health Worker certification is highly desirable.
