How you will contribute: 

The Nurse Practitioner plays a key part in determining continued eligibility for hospice care by conducting clinical assessments. This role includes performing face‑to‑face visits with hospice patients and collaborating closely with RN Case Managers, the interdisciplinary team, and the sponsoring physician to ensure all patient needs are met. 

This role requires compassion, strong clinical judgment, excellent communication skills, and the ability to support patients and families through highly sensitive and meaningful experiences. 

How you will make an impact:  

•  Conduct Medicare-required face to face encounters prior to the 180th day recertification and at each subsequent recertification, confirming that the encounter occurred under physician supervision. 

•  Document clinical findings from face to face visits, including: 

  • Evidence of decline in clinical status
  • Evidence of non–disease-specific indicators (e.g., PPS, BMI, FAST scale, ADL dependence)
  • Relevant co morbid conditions

•  Communicate all clinical findings to the physician responsible for recertifying the patient. 

•  Complete and submit attestation statements confirming the date of each face to face encounter. 

•  Submit accurate time and mileage logs for all patient related activities to the Medical Director every two weeks. 

•  Ensure the consistent use of the 4Ms—What Matters, Medications, Mentation, and Mobility—to provide Age Friendly Care.

The Experience and Skills You’ll Bring

  • Master’s Degree in Nursing 
  • Current unrestricted Nurse Practitioner license in the state of practice 
  • Board Certification 
  • Hospice experience preferred. 
  • Geriatrics experience preferred.
  • Must maintain a valid driver’s license and good driving record
