Are you an organized, compassionate healthcare professional who thrives in a fast-paced environment? Do you have experience with skilled nursing or long-term care admissions and enjoy bringing people and teams together?


We’re looking for a Intake Coordinator to be the critical first step in connecting patients with the right care.


In this role, you’ll serve as the primary point of contact for new patient referrals, coordinating with facilities, clinical teams, financial partners, and admissions professionals to ensure referrals are processed accurately, efficiently, and with the patient’s needs at the center of every decision.


Essential Duties:

  •  Serve as the primary point of contact for new patient referrals.
  • Review referral documentation and screen patients to determine admission eligibility.
  • Coordinate the appropriate distribution of viable referrals to facilities that best meet each patient's needs.
  • Partner closely with Clinical and Financial teams and facility admissions teams.
  • Build strong, collaborative relationships with facility and admissions teams.
  • Ensure referrals are processed promptly, accurately, and confidentially.


Requirements

 Requirements:

  • Associate's degree in business or health related field preferred
  • 2 years' experience in long term/ skilled care admissions preferred
