There are times whenĀ additionalĀ medical supervision and support are needed on our Campus. The LVN-PRN provides short-term, as-needed medical coverage to help ensure the health, safety, and well-being of the individuals we serve. This position works collaboratively withĀ BVĀ staff and leadership to respond to medical needs, provideĀ appropriate firstĀ aid and emergency care within the scope of LVN certification, andĀ assistĀ with assessing situations that may requireĀ additionalĀ medical attention.Ā
TheĀ LVN-PRN serves as a calm, compassionate, and professional medical resource while interacting respectfully with individuals with IDD, their families,Ā and BVĀ team members. The position may be scheduled forĀ special events, periods of increased medical needs, staff shortages, or other short-term campus coverage need.Ā
The ideal candidate is dependable, compassionate, observant, and comfortable working with adults with IDD. TheĀ LVN-Ā PRN must be able to respond appropriately to medical concerns, communicate effectively withĀ the BV team,Ā maintainĀ appropriate documentation, and followĀ BV policies and procedures.Ā
This is a PRN, short-term position with hours based on the medical coverage needs of the Breckenridge Village campus.Ā

Essential Functions:Ā
Perform nursing dutiesĀ in accordance withĀ nursing guidelines and best practicesĀ establishedĀ by the Texas Board of Nursing for Licensed Vocational NursesĀ
Perform the following duties to ensure the highest level of medical care for each resident assigned to you:Ā Ā
a. Respond to injuries and illness during normal business hoursĀ Ā
b. Evaluate all health and medical complaints of residents and coordinateĀ appropriate careĀ forĀ follow upĀ Ā
c. Regarding medication for assigned residents: Manage medication ordering and delivery,Ā ensureĀ accurate distribution of medications,Ā maintainĀ adequate supplies of medication,Ā ensureĀ proper storage of medications, Re-label any medication that is changed with updated label,Ā performĀ audits of MAR, as directedĀ train house managers on medication administration or any other medical function necessary including any necessary training on assistance with activitiesĀ of daily livingĀ
Maintain medical documentation for each resident assigned:Ā Ā
a. Enter appointment summaries into Electronic Medical Record within 24 hours ofĀ the appointment, including any writtenĀ material orĀ oral instruction providedĀ at appointment.Ā Ā
b. Update medication administration records (MAR) in real time when medicationsĀ areĀ Ā Ā changed,Ā addedĀ orĀ discontinued.Ā Ā
c. CreateĀ nurse progress note, in real time, in Electronic Medical Record when anyĀ communicationĀ regardingĀ medical care is received from or provided to a resident,Ā guardian, or Breckenridge Village staff memberĀ Ā
d. Document health and medical complaintsĀ
Communicate in a clear, transparent, andĀ timelyĀ manner all informationĀ regardingĀ each resident assigned to you:Ā Ā
a. Contact the QIPD immediately when any of the following occur: Illness or injury requiring transport to Urgent Care or Emergency Department Illness that requires a medical appointment or isolation/quarantine Injury that requires medical attention or A medication change is ordered for resident.Ā Ā
Contact the parent/guardian within the same business day whenever possible but never more than 24 hours post appointment, to discussĀ medicalĀ appointments, review documentation and instructions, Medication changes, Illnesses or injuries requiring transport to Urgent Care or Emergency Department, Illness that requires a medical appointment or isolation/quarantine orĀ injury that requires medical attentionĀ
Must be able to perform physical activity such as extensive walking, lifting of patients,Ā stretchingĀ and bendingĀ
Report signs of abuse, neglect, and exploitation and sexually provocative behavior and implement professional response, reporting and documenting proceduresĀ
Adhere to professional and ethical standards of conduct outlined in Breckenridge Village personnel polity manual includingĀ demonstratingĀ respect for agency staff, residents, and community members andĀ complying withĀ required dress code at all times.Ā
Provide 1:1 (one to one) supervision for residents when medically necessary on our campus, in a hospital or other locationsĀ as needed.Ā
While providing 1:1 (one to one) supervision for residents, our LVN-PRN will act as an advocate on the residentās behalf.Ā Ā

Measurable Deliverables:Ā
1. Meet all state regulatory guidelines and standards thatĀ are applicableĀ to this position, ensuring no deficiencies or concerns are foundĀ regardingĀ medical care during annual fundamental surveys.Ā
2.Ā MaintainĀ resident physical and mental safety by making immediateĀ reportĀ of any changes inĀ residentĀ health of behavior to the Nurse Manager and the QIDP with whom you collaborateĀ
3. Provide the highest level of resident careĀ
a. Daily reviewĀ ofĀ progress notesĀ and act asĀ required,Ā
b. Daily review ofĀ incident reportsĀ andĀ act asĀ requiredĀ
c. Daily review of communication from residents, parents/guardians, QIDP or any other team member and act asĀ required,Ā
d. Address each medical concern with urgencyĀ
e. Treat resident medical needs as though you would for your own familyĀ
f. In emergency situations, call 911 firstĀ
4. Ensure proper time management of all daily, weekly, monthly, quarterly, and annual tasksĀ
5. Document all resident health information as close to real time as possible but not later thanĀ 24 hoursĀ
