Adult Health Homes - Care Management Supervis

Summary:  The Adult Health Home Care Management Supervisor is an integral part of the Care Management team. The Adult Health Home Care Management Supervisor oversees care management staff and all activities related to the development and implementation of individualized plans of care. The Supervisor adheres to and promotes the philosophy and mission of the company by performing the following duties.

Duties and Responsibilities:  

1. Responsible for day-to-day oversight of care management staff and their duties: including coverage during the absence of Adult Care Coordinators. If providing this coverage: is responsible for conducting monthly visits to meet with assigned clients. Requires traveling to these face to face contacts at a frequency based on a case acuity.

2. Directly supervises 2+ employees in the Adult Care Management team. Carries out supervisory responsibilities in accordance with the organization's policies and applicable laws. Responsibilities include interviewing and training employees; planning, assigning, and directing work; appraising performance; addressing complaints and resolving problems.

3. Assists with overall planning and direction from the Adult Health Home Program Manager.

4. Reviews enrollment referrals with Adult Health Home Program Manager and assigns cases to Adult Care Coordinators.

5. Organizes workflow and ensures staff have a working understanding of duties and tasks.

6. Monitors staff productivity and provides guidance and coaching.

7. Responsible for interviews, hiring and providing proper training of department staff.

8. Provides supervision, case reviews and completes employee evaluations.

9. Works with Program Manager to create a process for time lines and quality assurance.

10. Identifies and utilizes both formal and informal community supports and networks.

11. Ensures adherence to all Department of Health and Health Home guidelines and standards.

12. Attends agency and Health Home trainings and ensures all staff are up to date on necessary trainings.

13. Works collaboratively with Adult Health Home Program Manager and Care Management Outreach Manager to ensure positive communication of program operations.

14. Works with identified staff trainers to ensure new staff onboarding and training is comprehensive and accurate.

Cleared with the New York State Central Registry and Department of Health.

Valid NYS Driver's License is required.

Education/Experience

• Bachelor’s Degree in Human Services, Social Services, Psychology or related field with at least 2 years experience providing service coordination for community-based services to individuals, preferably adults.
