Job Title: Activity Facilitator 

Home Office: Autumn Ridge Supportive Living - Vienna, IL 

Area of Focus: Community Collaboration 

Program(s): Supportive Living Program - SLP helps adults age 65+ live independently by providing them with housing in an apartment-style setting that emphasizes privacy and autonomy, while also offering services that support their health and well-being and providing a little extra assistance with the activities of daily life.

Immediate Supervisor: Assistant Director 

Hours of Work: Full-time, 35 hours/week. 

FLSA Classification: Non-exempt

Activity Facilitator Minimum Qualifications: High School Diploma/GED; Illinois Certified Nursing Assistant (CNA).

Activity Facilitator Skills & Knowledge: Knowledge of geriatrics & service care planning, ability to relate to senior adults, knowledge of local health and human services resources, excellent written and oral communication skills, basic computer skills.

Activity Facilitator Special Requirements: Maintains a valid driver’s license and vehicle liability insurance. Operates agency vehicles and personal vehicles while driving for agency business in a safe and lawful manner at all times. Maintain First Aid & CPR certification. Must complete required registry clearances and criminal fingerprint background checks. Maintain current Illinois C.N.A. certification. Must be at least 18 years old. 

Activity Facilitator job duties include, but are not limited to:

  • Answers phones in a polite, helpful manner and directs calls or messages to the appropriate staff or resident.
  • Scan and file resident accident and incident reports. 
  • Scan and ensure all documentation is in the resident’s EMAR.
  • Transport residents when needed for doctor appointments and shopping.
  • Distributes agency and resident’s mail.
  • Assists with planning and coordinating resident events including seasonal and special events.
  • File monthly documents for nursing and kitchen departments.
  • Manage and inventory activity and facility supplies.
  • Attend off-site activities with residents.
  • Assists with calendar creation and flyers.
  • Reminds residents of events and activities.
  • Execute scheduled activities for residents.
  • Housekeeping duties when needed or necessary.
  • Stay up to date on trends and best practices for activity programs.
  • Lead exercising activities with residents.
  • Promote and market programs and activities to increase participation.
  • Understand resident preferences and incorporate suggestions into the activity calendar.
  • Assist volunteers as needed.
  • Required to be awake, alert, and attentive at all times while on shift. Sleeping on shift is not permitted at any time and may be considered neglect in some circumstances.

Activity Facilitator Salary Range: $19.30 - $19.55 per hour
