About Little Flower

Little Flower Children and Family Services of New York is a human services nonprofit supporting the well-being of children, youth, families, and individuals across New York City and Long Island since 1929.

Our staff of more than 500 builds well-being by providing prevention services, foster care, residential treatment care, adoption services, programs and services for individuals with developmental disabilities, and medical and mental health services.


Principle Responsibilities


Seeking an intern to provide screening and navigation within the Social Care Network (SCN) program. SCN connects individuals to critical community support services to address non-medical health-related social needs, such as housing, food, and transportation. 


Social Care Network bridges the gap between clinical care and community services through:

  • Core Services: SCN connects eligible individuals to free resources like medically tailored meals, housing application assistance, utility support, and reliable transportation to essential non-medical appointments.
  • Screening & Navigation: Care providers, managed care plans, or social care navigators screen individuals for unmet needs. Eligible participants are then guided to temporary or long-term community-based organizations.

Learning Outcomes

  • Gain experience navigating community resources and referral systems by connecting individuals with appropriate community-based organizations and support services within the Social Care Network.
  • Build an understanding of the relationship between social determinants of health and health outcomes by observing how community resources and coordinated care can improve individuals’ overall well-being and access to services.

Required Qualifications and Experience

  • Currently enrolled in a degree program in Social Work, Psychology, Sociology, Human Services, Education, or related field.
  • Interest in working with youth involved in foster care or prevention services.
  • Energetic, dependable, and emotionally mature.
  • Valid NYS driver’s license or NYS Non-Driver ID

Travel Requirements

This internship is based in Wading River, NY.
