What You'll Do
- Perform comprehensive physical therapy evaluations and establish individualized plans of care.
- Develop and implement treatment plans to improve mobility, strength, balance, and functional independence.
- Monitor patient progress and modify treatment plans to achieve optimal outcomes.
- Educate patients, families, and caregivers on exercises, safety, and disease management.
- Collaborate with physicians, nurses, and the interdisciplinary care team to coordinate patient care.
- Complete accurate and timely clinical documentation.
- Maintain compliance with infection control standards, agency policies, and regulatory requirements.
Qualifications
- Graduate of an accredited Physical Therapy program.
- Current, unrestricted Physical Therapist (PT) license.
- Current CPR certification.
- Valid driver's license, reliable transportation, and current auto insurance.
- One (1) year of Home Health experience preferred.
- HomeCare HomeBase experience preferred.
Physical Requirements
- Ability to stand, walk, bend, squat, kneel, climb, and sit for extended periods.
- Ability to safely lift, carry, push, or pull up to 100 pounds with appropriate assistance when necessary.
