Job Description:
- Complete comprehensive patient admission assessments and documentation.
- Perform medication reconciliation and identify potential medication-related concerns.
- Coordinate and facilitate timely patient admissions and discharges.
- Provide individualized patient and family education regarding hospitalization, care plans, and discharge instructions.
- Collaborate closely with physicians, case managers, nursing staff, and ancillary departments to support effective care transitions.
- Advance patient throughput initiatives and care coordination efforts.
- Help reduce administrative demands on bedside nursing teams, creating greater capacity for direct patient care.
- Promote a culture of safety, efficiency, and patient-centered excellence throughout the hospitalization journey.

- Graduate of an accredited or state-approved Registered Nursing program.
- Current Massachusetts Registered Nurse (RN) license.
- Two years of clinical nursing experience.
- Strong clinical assessment, communication, and organizational skills.
- Demonstrated commitment to providing safe, compassionate, high-quality patient care.
- Bachelor of Science in Nursing (BSN) strongly preferred.
- Experience in one or more of the following areas:
- Medical-Surgical Nursing
- Care Coordination
- Transitions of Care
- Patient Flow or Throughput Operations
- Ability to effectively balance exceptional patient care with operational efficiency and patient movement initiatives.
- Current Massachusetts Registered Nurse (RN) license required.

Pay Range:
$36.00 - $90.14The pay range listed for this position is the base hourly wage range the organization reasonably and in good faith expects to pay for this position at this time. Actual compensation is determined based on several factors, that may include seniority, education, training, relevant experience, relevant certifications, geography of work location, job responsibilities, or other applicable factors permissible by law. Compensation may exceed the base hourly rate depending on shift differentials, call pay, premium pay, overtime pay, and other additional pay practices, as applicable to the position and in accordance with the law.
