Purpose: PACT (Program of Assertive Community Treatment) nurses provide integrated psychiatric and medical nursing services in the community to help adult individuals with serious mental illness maintain stability, safety, wellness, and independence. What are we looking for in a Registered Nurse? The Registered Nurse provides general nursing services to clients and nursing directions to staff. Client care is provided as a member of a multidisciplinary team. We are also looking for a nurse who is experienced working with vulnerable populations and is open to traveling throughout the community.   Registered Nurse duties may include:   Medication Management: Administer prescribed medications and long-acting injectable medications; assist with medication organization and delivery; assess adherence; provide medication education; and monitor therapeutic response, adverse effects, and potential medication interactions. Psychiatric Assessment and Monitoring: Assess and monitor mental status, mood, psychosis, behavior, sleep, anxiety, substance use, level of functioning, and changes from the client’s established baseline. Communicate significant changes to the treatment team and psychiatric provider. Medical Assessment and Health Monitoring: Monitor physical health conditions, vital signs, weight, metabolic concerns, and other medical needs. Identify symptoms requiring primary care, urgent care, emergency evaluation, or additional medical follow-up. Laboratory and Medication-Specific Monitoring: Coordinate and follow required laboratory testing and medication monitoring, including lithium levels, valproate levels, clozapine/ANC monitoring, metabolic labs, and other testing based on prescribed medications and clinical needs.  Crisis Assessment and Intervention: Assess clients experiencing acute psychiatric or medical changes, including suicidal or homicidal ideation, grave disability, worsening psychosis, intoxication or withdrawal, medication reactions, and other safety concerns. Coordinate crisis services or a higher level of care when indicated. Care Coordination: Coordinate care with psychiatric providers, primary care providers, pharmacies, hospitals, emergency departments, inpatient units, Designated Crisis Responders (DCRs), caregivers, and other members of the interdisciplinary PACT team. Community-Based Nursing and Outreach: Provide nursing services in the setting most appropriate for the client, including the client’s home, shelter, community, clinic, hospital, or other community location. Support clients who may have difficulty consistently accessing traditional office-based healthcare. Health Promotion and Client Education: Provide education regarding medications, psychiatric and medical diagnoses, nutrition, sleep, substance-use risks, smoking cessation, chronic disease management, preventive care, and strategies that support overall wellness. Therapeutic Engagement and Advocacy: Develop therapeutic relationships with clients who may have complex needs or difficulty engaging in treatment. Advocate for appropriate psychiatric, medical, social, and community resources while promoting client autonomy and recovery. Documentation and Team Communication: Accurately document assessments, medication administration, interventions, client response, safety concerns, provider notifications, care coordination activities, and follow-up plans. Communicate clinically significant information to the interdisciplinary team. Core Nursing Functions:   Utilizes the nursing process, providing a systematic approach to client health needs through assessment, nursing diagnosis, planning, implementation, and evaluation. Carries out orders as prescribed by staff providers in a concise and timely manner. Establishes and maintains professional relationships with clients and families, providing information, education, care coordination, and support. • Implements prevention strategies for various health conditions as appropriate. • Actively engages in treatment teams and team-oriented strategies to achieving care objectives. • Utilizes a variety of resources to increase clinical and professional competence. • Delegates nursing tasks to appropriate staff members, training and supervising them to ensure accuracy and consistency in care and reporting. • Represents and promotes professional nursing in all settings. • Provides support to clients with activities of daily living (ADLs) including but not limited to eating, drinking, dressing, bathing, and toileting. Crisis Evaluation Intervention:   Utilizes appropriate crisis intervention techniques for client stabilization. Accurately screens for risk to self, others, property and the presence of grave disability and makes appropriate referrals. Documents and reports and/or notifies as required by law Applies QPRT (Question, Persuade, Refer or Treat) principles in assessing risk and in safety and treatment planning. Develop and documents appropriate safety plans.   Injections:   Uses Standard Precautions and performs hand hygiene before and after administering an injection.Verifies the medication orders are correct and match the provider’s last note. Verifies the medication is available at the pharmacy or the client will be bringing the medication with them. Practices the Five Rights of Medication Administration; Right Patient, Right Medication, Right Dose, Right Route and Right Time. Demonstrates clean, safe and proper technique with administering and injection; hand hygiene, use of appropriate personal protection equipment (PPE). • Discards the needle and syringe in the appropriate receptacle. • Evaluates the client for adverse side effects or reactions following the injection. • Assesses client’s mood, behaviors, appearance, sleep, appetite and suicidal/homicidal risks factors. • Reschedules the client for their next injection. • Documents the injection into the client’s electronic health record.   Service Notes:   Completes all required service notes according to Comprehensive Healthcare’s guidelines, policies and procedures regarding format, content and timeliness. Language used in service notes is respectful, clear, descriptive, concise and client centered. Service notes accurately depict treatment modalities and interventions provided within the clinician’s scope of practice and according to treatment plan. Service notes accurately reflect and depict client status and progress consistent with assessment and reassessment data, problem statements and treatment plan goals and objectives.   Symptom & Medication Response Monitoring:   Conducts and documents ongoing assessment of client symptoms and their response to prescribed medications. Provides support and education to encourage clients to take medications as prescribed. Promotes illness self-management skills to assist clients with medication adherence, identifying warning signs and symptoms, and symptom management skills. Monitors medication side effects and facilitates access to, and communication with, medical staff as needed.       Qualifications: Associate or bachelor’s degree in nursing Active RN or LPN license in Washington or Compact State Must have valid driver's license and vehicle liability insurance In compliance with state and federal law, Comprehensive Healthcare requires all candidates to complete a post-offer, pre-employment drug screen and background check.
