This RN/LPN position supports our Opioid Treatment department, a new program which will help us increase access to evidence-based treatment for opioid use disorders to some of the most vulnerable members of our community. The program currently provides medication assisted treatment daily to clients and services to other hard-to-access populations throughout the greater Yakima community. This position will provide treatment daily to individuals in jail, and soon in a mobile medication unit in the community. Support is also provided to the outpatient opioid treatment program, which doses methadone. Position Details: Schedule:聽Monday-Friday 6a-2:30p. 1 Saturday shift required per month as needed. Pay Ranges: $41.79- $62.69 $15,000 Sign on Bonus Health Benefits Responsibilities: 聽 Core Nursing Functions: Utilizes the nursing process, providing a systematic approach to client health needs through assessment, nursingdiagnosis, planning, implementation, and evaluation. Carries out orders as prescribed by staff providers in a concise and timely manner. Establishes and maintains professional relationships with clients and families, providing information, education, carecoordination, and support. Implements prevention strategies for various health conditions as appropriate. Actively engages in treatment teams and team-oriented strategies to achieving care objectives. Utilizes a variety of resources to increase clinical and professional competence. Delegates nursing tasks to appropriate staff members, training and supervising them to ensure accuracy andconsistency in care and reporting. Represents and promotes professional nursing in all settings. Crisis Evaluation Intervention: Utilizes appropriate crisis intervention techniques for client stabilization. Accurately screens for risk to self, others, property and the presence of grave disability and makes appropriatereferrals. Documents and reports and/or notify as required by law. Applies QPRT (Question, Persuade, Refer or Treat) principles in assessing risk and in safety and treatment planning. Develops and documents appropriate safety plans. Employment Practices: Effectively and legally interviews applicants and makes appropriate staff selections. Follows policies and procedures for corrective action relating to staff performance. Provides sound supervision to all direct reports which include appropriate and frequent performance feedback. Supports employees in skill development and goal attainment. Completes performance evaluations in a timely and thorough manner. Appropriately utilizes the Human Resources Department as needed and required. Injections: Uses Standard Precautions and performs hand hygiene before and after administering an injection. Verifies the medication orders are correct and match the provider鈥檚 last note. Verifies the medication is available at the pharmacy or the client will be bringing the medication with them. Practices the Five Rights of Medication Administration; Right Patient, Right Medication, Right Dose, Right Route and Right Time. Demonstrates clean, safe and proper technique with administering and injection; hand hygiene, use of appropriate personal protection equipment (PPE) Discards the needle and syringe in the appropriate receptacle. Evaluates the client for adverse side effects or reactions following the injection. Assesses client鈥檚 mood, behaviors, appearance, sleep, appetite and suicidal/homicidal risks factors. Reschedules the client for their next injection. Documents the injection into the client鈥檚 electronic health record. Medical Supervision: Conducts and documents on-going medical supervision consistent with Comprehensive Healthcare鈥檚 policies and procedures, and consistent with the requirements for the supervised employee鈥檚 professional credentials. Assesses medical skills, gives constructive feedback, and provides (or arranges for) training and coaching to promote the development of technical skills, therapeutic approaches, and strength-based interventions appropriate for the client population served. Creates a continuing education plan and designs and tracks learning objectives to support new and ongoing skill development consistent with job requirements. Provides direction, supervision, coaching, training and regular performance feedback to build employee skills and support progress toward goals. Effectively and legally interviews applicants and makes appropriate staff selections. 路 Creates training and development plans to support new employees to be successful. 路 Completes performance evaluations in a timely and thorough manner. Assists with the development of the team budget and monitors budget throughout the year. Monitors program plan adherence to ensure quality service delivery. Collaborates with the program Director and Human Resources Department on hiring, retention and progressive discipline issues. Communicates relevant matters to direct reports timely and effectively. Medication Administration and Monitoring: Safely administers and documents medication according to provider orders and agency policies. Conducts and documents ongoing evaluation of intended effects, side effects, adverse reactions and medication non-compliance. Communicates this information to the appropriate parties. Implements appropriate interventions regarding medications and side effects. Provides medication education to client and family, including purpose of medication, side effects, dosage, frequency and duration. Encourages and supports clients in taking medications as prescribed. Promotes illness self-management skills. Medication Assisted Treatment Nursing Functions: Demonstrates an understanding of substance use disorders and their treatments and risks; promotes recoverythrough medication assisted treatment. Utilizes clinic security alarms appropriately and conducts regular tests as scheduled. Demonstrates the ability to utilize electronic dispensing software correctly. Evaluates client for signs and symptoms ofsedation or intoxication prior to administering methadone and/or suboxone. Demonstrates the ability to accurately dispense methadone and/or suboxone manually using pipettes and utilizes thismethod when appropriate. Follows protocols for receiving methadone/suboxone shipments and daily inventory ofmethadone/suboxone bottles. Immediately reports discrepancies to the Team Leader or designee. Ensures that take out medication is dispensed consistent with program policies. Follows program policies for medication spills or waste.
